Many agents are being considered for treatment of Parkinson disease (PD). Given the large number of agents and the limited resources to evaluate new agents, it is essential to reduce the likelihood of advancing ineffective agents into large, long-term Phase III trials. Futility design methodology addresses this goal.

Non-linearity of Parkinson's disease progression: implications for sample size calculations in clinical trials.

Background: Estimation of sample size for long-term studies of neuroprotection in Parkinson's disease requires information on expected clinical decline. Values may be obtained by analyzing existing long-term data sets or by prediction models of clinical decline applied to available data from shorter-term trials. The most commonly used measure to track clinical decline is the Unified Parkinson's Disease Rating Scale (UPDRS) but this measure is also affected by symptomatic therapy.

Haplotype insufficiency for suppressor of cytokine signaling-2 enhances intestinal growth and promotes polyp formation in growth hormone-transgenic mice.

GH may improve intestinal growth or function in patients with short bowel syndrome. Excessive trophic effects of GH or IGF-I may contribute to neoplastic growth or increased colorectal cancer risk in acromegaly. Identification of mechanisms that limit the tumorigenic potential of GH and IGF-I is desirable.

The effect of seawater composition and osmolality on hemolymph levels of methyl farnesoate in the green crab Carcinus maenas.

Green crabs, Carcinus maenas, exposed to dilute seawater (e.g., 5 ppt salinity, approximately 150 mOsm/kg) have hemolymph levels of methyl farnesoate (MF) that are up to 10-fold higher than animals in isosmotic seawater (27 ppt, approximately 800 mOsm/kg).

The North American Multiple System Atrophy Study Group.

The North American Multiple System Atrophy Study Group involves investigators in 12 US medical centers funded by a grant from the National Institutes of Health. The objectives are to examine the environmental and genetic risk factors for MSA; elucidate pathogenic mechanisms underlying the disorder; and refine evaluations used for assessment. During its first year, the group enrolled 87 patients, implemented four cores, and initiated four scientific projects.

Excessive daytime sleepiness and subsequent development of Parkinson disease.

Objective: To determine if excessive daytime sleepiness (EDS) can predate future Parkinson disease (PD).

Methods: EDS was assessed in 3,078 men aged 71 to 93 years in the Honolulu-Asia Aging Study from 1991 to 1993. All were free of prevalent PD and dementia.

Application of soft tissue modelling to image-guided surgery.

The deformation of soft tissue compromises the accuracy of image-guided surgery based on preoperative images, and restricts its applicability to surgery on or near bony structures. One way to overcome these limitations is to combine biomechanical models with sparse intraoperative data, in order to realistically warp the preoperative image to match the surgical situation. We detail the process of biomechanical modelling in the context of image-guided surgery.

Anisotropic multi-scale fluid registration: evaluation in magnetic resonance breast imaging.

Registration using models of compressible viscous fluids has not found the general application of some other techniques (e.g., free-form-deformation (FFD)) despite its ability to model large diffeomorphic deformations.

Verification of self-report of zygosity determined via DNA testing in a subset of the NAS-NRC twin registry 40 years later.

The National Academy of Sciences -- National Research Council (NAS-NRC) twin panel, created nearly 50 years ago, had twin zygosity determined primarily via a similarity questionnaire that has been estimated to correctly classify at least 95% of twins. In the course of a study on the genetics of healthy ageing in the NAS-NRC twins, DNA was collected for genome-wide scanning and zygosity confirmation was examined in 343 participating pairs. The sample was supplemented from two other studies using NAS-NRC twins where one or both co-twins were suspected to have Alzheimer disease or another dementia, or Parkinson's disease.

Occupation and parkinsonism in three movement disorders clinics.

Background: Few occupational risk factors for Parkinson disease (PD) have been identified. Healthcare, teaching, and farming have been associated with increased risk, while welding has been proposed to accelerate age at PD onset. The aim of the present study was to investigate occupational associations with PD or parkinsonism drawing from three different movement disorders clinics.

Comparison of maturation ponds and constructed wetlands as the final stage of an advanced pond system.

The treatment performance of a maturation pond (MP), the typical final polishing stage of an Advanced Pond System (APS), is compared with that of a surface-flow constructed wetland (CW) over 19 months. Both received approximately 67 mm d-1 of wastewater after passage through upstream stages of the APS. The MP, with greater sunlight exposure, had higher algal biomass (and associated suspended solids) than the CW, showed higher dissolved oxygen (DO) concentrations and greater diurnal variation in DO and pH.

Potential biogas scrubbing using a high rate pond.

The potential to scrub biogas in a high rate pond (HRP) was evaluated using apparatus designed to maximize gas-liquid contact. Experiments compared the removal of carbon dioxide from synthetic biogas by an "in-pond angled gutter" to that by a simulated "counter-current pit." Results showed that the counter current pit has potential for use in biogas scrubbing, with synthetic biogas carbon dioxide composition consistently reduced from 40% to < 5%.

Quality of life in Essential Tremor Questionnaire (QUEST): development and initial validation.

Background/objective: Essential tremor (ET) can diminish functioning and quality of life (QOL) but generic QOL measures may be relatively insensitive to ET and its therapies. We sought to develop an ET-specific measure that might be more sensitive, acceptable to patients, relatively brief, and easily used.

Design/methods: A sample of 200 patients (average age 70 years, range 30-91; average disease duration 15 years) rated the extent to which tremor impacts a function or state, tremor severity in various body parts, perceived health, and overall QOL.

A comparison of methods for estimating the lactate threshold.

The purpose of this study was to examine the accuracy of tests that may be used by distance runners to estimate the lactate threshold. Competitive distance runners/triathletes (N = 27) performed a criterion test that directly measured (blood lactate of 4.0 mmol.

Effect of reproductive factors and postmenopausal hormone use on the risk of Parkinson disease.

Objective: Parkinson disease (PD) is less common in women possibly because of hormonal or reproductive influences. The objective of this study was to evaluate the associations of reproductive factors and postmenopausal hormone use with the risk of PD among postmenopausal women.

Methods: Incident cases (n = 178) and randomly selected age-matched controls (n = 189) who were members of the Kaiser Permanente Medical Care Program (KPMCP) of Northern California participated in the study conducted during the years 1994 to 1995.

High-resolution spectroscopy with a femtosecond laser frequency comb.

The output of a mode-locked femtosecond laser is used for precision single-photon spectroscopy of 133Cs in an atomic beam. By changing the laser's repetition rate, the cesium D1 (6s 2S(1/2)-->6p 2P(1/2)) and D2 (6s 2S(1/2)-->6p 2P(3/2)) transitions are detected and the optical frequencies are measured with accuracy similar to that obtained with a cw laser. Control of the femtosecond laser repetition rate by use of the atomic fluorescence is also implemented, thus realizing a simple cesium optical clock.

Influence of biofilm on removal of surrogate faecal microbes in a constructed wetland and maturation pond.

The effect of biofilm on the attenuation of pathogen-sized particles from wastewater was compared for biofilms cultivated in a surface flow constructed wetland (SFW) and maturation pond (MP) The fate of fluorescently labelled microspheres (FLM) as surrogates for viruses (0.1 microm), bacteria (1 microm) and parasitic protozoa (4.5 microm dia) was investigated in microcosms in the presence or absence of biofilms.

Despite the importance of randomized, dose-response studies for proper evaluation of effective clinical interventions, there have been no dose-response studies on the effects of exercise amount on abdominal obesity, a major risk factor for metabolic syndrome, diabetes, and cardiovascular disease. One hundred seventy-five sedentary, overweight men and women with mild to moderate dyslipidemia were randomly assigned to participate for 6 mo in a control group or for approximately 8 mo in one of three exercise groups: 1) low amount, moderate intensity, equivalent to walking 12 miles/wk (19.2 km) at 40-55% of peak oxygen consumption; 2) low amount, vigorous intensity, equivalent to jogging 12 miles/wk at 65-80% of peak oxygen consumption; or 3) high amount, vigorous intensity, equivalent to jogging 20 miles/wk (32.
