A 21-month-old patient diagnosed as 18 trisomy, having large ventricular septal defect with pulmonary valve stenosis, and gastroesophageal reflux disease, was admitted for laparoscopic Nissen fundoplication and gastrostomy. Anesthesia was induced with midazolam, fentanyl, and rocuronium, and maintained with sevoflurane and remifentanil. After induction of anesthesia, oxygenation became worse, possibly due to diminished pulmonary blood flow, which was partially resolved with vasopressor administration and fluid infusion.

Our aim was to investigate how patients recognize a flare-up of UC and how significant a gap there is between symptoms recognized as a flare-up and symptoms judged to require physician visit.

Objective: Acidosis produces a negative inotropic effect on cardiac muscle against which catecholamines and phosphodiesterase III inhibitors have limited therapeutic effects. Background: The significance of diffusion-weighted imaging (DWI) is uncertain for the diagnosis of nodal involvement. The purpose of this study was to examine diagnostic capability of DWI compared with PET-CT for nodal involvement of lung cancer.
Methods: A total of 160 lung cancers (114 adenocarcinomas, 36 squamous cell carcinomas, and 10 other cell types) were analyzed in this study.
Background: Thirty to forty-five percent of ulcerative colitis (UC) patients show non-adherence to aminosalicylates, and non-adherence has been reported to increase the risk of clinical relapse. Because Japan differs from Western countries in terms of health care and drugs administered, adherence to aminosalicylates in Japan may differ from that elsewhere. Therefore, we examined aminosalicylate adherence and its relationship to the risk of clinical relapse of UC in Japan.

We experienced a case of ulnar nerve palsy after abdominal surgery under general anesthesia in a patient with a history of total elbow arthroplasty and ulnar nerve translocation. It appears that pressure on medial epicondyle caused the palsy and that the position of the ulnar nerve has to be carefully examined before surgery in a patient with a history of total elbow arthroplasty and ulnar nerve translocation.

Methods: EORTC quality of life (QOL) translation guidelines were followed to create a Japanese version of the EORTC QLQ-HCC18. This was then administered to 192 patients with hepatocellular carcinoma along with the EORTC QLQ-C30 and FACT-Hep questionnaires.
Background: The "BURP" maneuver (back, upward, right lateral, pressure) improves the visualization of the larynx for experienced anesthesiologists during orotracheal intubation in patients with difficult airway. We investigated whether this maneuver has same efficacy for inexperienced residents in anesthesiology.
Methods: We compared the visual condition of the larynx using "BURP" maneuver, back pressure or no maneuver between 37 residents and 16 staff anesthesiologists in 132 patients receiving general anesthesia.
Purpose: The aim of this study was to evaluate health-related quality of life in patients with a colostomy immediately before and during the first year after surgery.
Subjects And Setting: Patients (aged ≥20 years) who were diagnosed with rectal cancer and scheduled to undergo curative surgery with a permanent colostomy were recruited for this study. Data were collected at 2 university hospitals in Tokyo.
Stiff-person syndrome is an uncommon disease characterized by muscular rigidity and painful spasms in the axial and limb muscles. We report a 58-year-old woman with stiff-person syndrome undergoing thymectomy under general anesthesia. Before surgery, her medications were 25 mg of diazepam, 2 mg of clonazepam, and 15 mg of gabapentin per day.

Background: The usual mechanism of anterior shoulder dislocation is widely believed to be a combination of glenohumeral joint abduction, extension, and external rotation forces, even though no published reports to date have investigated the arm position of anterior shoulder dislocation in detail. Understanding the exact position of anterior shoulder dislocations is important for the management of anterior shoulder instability.

Aminosalicylates are effective in inducing remission and are also useful in preventing relapse of ulcerative colitis (UC); however, previous studies have reported a 30%-45% rate of nonadherence to prescribed medication among UC patients. Enumerating and conceptualizing the difficulties in taking aminosalicylates enable us to assess and support patients more effectively. The aim of this study was to investigate the difficulties in taking aminosalicylates among Japanese UC patients and explore factors related to these difficulties.
