When a primary tooth sustains a traumatic injury, development of the permanent tooth can be disturbed, leading to various malformations. This case report details the 7-year follow-up of a developmental anomaly of a central mandibular incisor in a 10-year-old Japanese girl with a history of dental trauma that had occurred at age 4. The trauma had resulted in unusual crown dilaceration in the permanent successor, which exhibited a discoloured and abnormal crown morphology.

Periradicular surgery is often used as a last resort to save an endodontically treated tooth with a persistent periapical lesion. The introduction of surgical microscopes, ultrasonics, and compatible root-end filling materials has made periradicular surgery a much more predictable treatment. The advantages of modern periradicular surgery include easier identification of root apices, smaller osteotomines, shallower resection angles, and tight sealing within the prepared root-end cavity.

This case report presents the endodontic treatment of a maxillary right lateral incisor with a perforating internal resorption in a 50-year-old woman. Radiographically, internal resorption appears as a fairly uniform, radiolucent enlargement of the pulp canal and distortion of the original root canal outline. The use of cone beam computed tomography can help the clinician in making a confirmatory diagnosis and determining the treatment plan before undertaking the actual treatment.

We describe the successful use of a combination of nonsurgical root canal treatment and ultrasonic irrigation for collaborative management of a maxillary left lateral incisor with perforation of the apical third of the root. During the endodontic treatment procedure, the ultrasonically activated tip was used for intracanal irrigation. The area of perforation in the apical third of the root and the main root canal space were obturated with gutta-percha and root canal sealer, using a lateral condensation method.

Dens evaginatus is a developmental anomaly characterized by the presence of an accessory cusp composed of enamel and dentine, usually containing pulp tissue. This condition is clinically important because of fracture or wear of the tubercle, which can frequently lead to the major complication of pulp necrosis and periapical infection. Treatment varies according to pulp condition, tubercle integrity, and stage of root development.
