Article Synopsis
  • WNV NID leads to high mortality and disability, requiring long ICU stays, which were studied in patients treated at a Croatian hospital from 2013-2018.
  • Out of 23 patients (mostly older males), ICU mortality was low (8.7%), but many still had moderate to severe disability at discharge.
  • At long-term follow-up, 30.5% of patients died, while some improved their functional status, indicating that intensive treatment followed by rehab can help recovery in severe cases.

Data on the immune response to West Nile virus (WNV) are limited. We analyzed the antiviral cytokine response in serum and cerebrospinal fluid (CSF) samples of patients with WNV fever and WNV neuroinvasive disease using a multiplex bead-based assay for the simultaneous quantification of 13 human cytokines. The panel included cytokines associated with innate and early pro-inflammatory immune responses (TNF-α/IL-6), Th1 (IL-2/IFN-γ), Th2 (IL-4/IL-5/IL-9/IL-13), Th17 immune response (IL-17A/IL-17F/IL-21/IL-22) and the key anti-inflammatory cytokine IL-10.

Article Synopsis
  • * During the outbreak, tests were conducted on 246 patients for neuroinvasive and febrile diseases, confirming WNV cases across ten counties and indicating new areas affected by human cases for the first time.
  • * Phylogenetic analyses indicated the circulation of WNV lineage 2, and testing of horses and poultry showed the presence of WNV antibodies, alongside the detection of viral RNA in local birds and mosquitoes, emphasizing the need for ongoing 'One Health' surveillance.
