[Reconstructive-restorative microsurgical operations in cicatricial ureteral stenosis of tuberculous etiology].

The authors describe a new method of operative intervention with cutting a flap out of the urinary bladder wall. The urinary bladder is anastomosed with the ureter with the help of nodular microsurgical sutures put from the outside, the antireflux protection being realized distal to the zone of anastomosis by the formation of two urinary bladder flaps and their suture "cross by cross" in front of and behind the base of the created urinary bladder tube. Experimental and clinical materials showing the effectiveness of the proposed treatment are described.

[Status and prospects of the problem of endemic goiter].

The paper is concerned with the clinical problems of endemic goiter control. The stabilization of prevalence of thyroid hyperplasia has been shown as a result of underestimation of other than iodine deficiency giterogenic factors. Functional essence of the disease, a tendency to hypothyrosis, have been unraveled.

[Postinjection changes in angiopathy patients].

The results are presented of studying the response of the foot skin blood microcirculatory vessels after injecting 2ml of saline solution of NaCl into the hip. A group of patients with diabetes mellitus aggravated by angiopathy of low extremities was managed with insulin during 4 to 18 years and examined versus insulin-untreated diabetes mellitus patients, persons with low limb obliterating thromboangiitis and healthy volunteers. The studies were carried out, using the "skin window" method, which allows one to investigate not only the capillary function but also the state of other skin microcirculatory units.

[Difficulties in the differential diagnosis of tuberculosis of the breast].

An analysis of the own material and of literature data permits to note alterations in the clinical course of mammary gland tuberculosis, which is the manifestation of tuberculosis pathomorphism under modern conditions. The lesion of the mammary gland is most commonly an isolated disease. This interferes with the differential diagnosis between tuberculous mastitis and tumors of the mammary gland and supports the opinion that the spreading of infection from other old tuberculous foci via hematogenic path plays the main part in the pathogenesis of tuberculosis of the mammary gland.

[Changes in the sensitivity of lipoatrophic tissues to insulin under the effects of novocaine].

A study was made of the isolated adipose tissue of 9 healthy women and the adipose tissue from the zone of lipoatrophy obtained from 7 women suffering from diabetes mellitus treated with insulin. In difference to the adipose tissue of healthy persons, in the lipoatrophic adipose tissue there occurred under the effect of insulin-novocain mixture a marked increase in the incorporation of C-14-labeled glucose into the total lipids of the adipose tissue. The data obtained served as an argument in favour of the neurotrophic genesis of the postinjection insulin lipoatrophies and proved the neuro-trophic mechanism of the therapeutic effect of the insulin-novocain mixture.
