Article Synopsis
  • Hermansky-Pudlak syndrome (HPS) is a rare genetic disorder that leads to progressive pulmonary fibrosis (HPS-PPF) in some cases, which has a high mortality rate and lacks effective treatments aside from lung transplantation.
  • A case study of HPS type 1 treated with long-term nintedanib showed promising results before lung transplantation, indicating its potential benefits for managing HPS-PPF.
  • The examination of the resected lungs revealed lesions consistent with fibrotic changes, supporting the idea that anti-fibrotic medications like nintedanib could be a safe and beneficial option for treating HPS-PPF.

A 40-year-old man with high fever, hemoptysis, and fatigue showed a 10-cm mass in the middle and lower lobes of the right lung on computed tomography. Histological examination of transbronchial biopsy specimens showed sheets of small round tumor cells and mild staining for CD99. Primary Ewing sarcoma was suspected, and a trimodality therapy consisting of chemotherapy, intensity-modulated radiation therapy, and right pneumonectomy with surrounding tissue resection was performed.
