Anorectal Abscess in a Patient with Neutropenia and Refractory Acute Myeloid Leukemia: To Operate or not to Operate?

BACKGROUND Anorectal infections occur in 5% to 9% of patients with hematological malignancies, including acute myeloid leukemia, and cause febrile neutropenia and sepsis. Surgical treatments of anorectal abscesses tend to be avoided in patients with leukemia owing to persistent neutropenia and bleeding risks. CASE REPORT A 56-year-old man presented with an ischiorectal abscess.

Cytotoxic and genotoxic profiles of benzo[a]pyrene and N-nitrosodimethylamine demonstrated using DNA repair deficient DT40 cells with metabolic activation.

Benzo[a]pyrene and N-nitrosodimethylamine are major genotoxic compounds present in cigarette smoke, food and oil. To examine the type(s) of DNA damage induced by these compounds, we used a panel of DNA-repair-pathway-deficient mutants generated from chicken DT40 cells and achieved metabolic activation of the test compounds by including rat liver S9 mix. Consistent with expections, benzo[a]pyrene and N-nitrosodimethylamine require metabolicactivation to become genotoxic.

Induction of aldo-keto reductases (AKR1C1 and AKR1C3) abolishes the efficacy of daunorubicin chemotherapy for leukemic U937 cells.

Continuous exposure to daunorubicin (DNR) confers resistance against the drug-elicited lethality of leukemic cells and then reduces the remission rate. However, the detailed mechanisms involved in resistance development of leukemic cells to DNR remain unclear. Upregulation of aldo-keto reductases (AKRs) in human leukemic U937 cells was evaluated by gene-specific PCR and western blot analyses, and the contribution of AKRs toward the DNR sensitivity was assessed using gene expression and RNA-interference techniques and specific inhibitors.

Distinctive flow pattern of wall shear stress and oscillatory shear index: similarity and dissimilarity in ruptured and unruptured cerebral aneurysm blebs.

Object: The difference in the hemodynamics of wall shear stress (WSS) and oscillatory shear index (OSI) between ruptured and unruptured aneurysms is not well understood. The authors investigated the hemodynamic similarities and dissimilarities in ruptured and thin-walled unruptured aneurysm blebs.

Methods: Magnetic resonance imaging-based fluid dynamics analysis was used to calculate WSS and OSI, and hemodynamic and intraoperative findings were compared.

Symptomatic vasospasm in elderly patients with aneurysmal subarachnoid hemorrhage: comparison with nonelderly patients.

Objective: To clarify the influence of age on the occurrence of symptomatic vasospasm (SVS), we retrospectively compared 34 elderly (over 70 years) and 71 nonelderly patients with aneurysmal subarachnoid hemorrhage (SAH).

Methods: Between 2008 and 2010, at our hospital 105 patients (Hunt and Kosnik grades I-IV) underwent aneurysm surgery within 72 h of the insult. They were divided into four groups based on their age (younger/older than 70 years) and treatment (aneurysmal clipping or coiling).

[Operated case of a ruptured aneurysm arising from fenestration of the vertebral artery].

Aneurysm arising from duplication of the vertebral artery (VA) is rare. We report a surgical case of an aneurysm of VA fenestration and provide a literature review. A 42-year-old man suffered from sudden onset of severe headache.

Ruptured peripheral aneurysms in a collateral pathway associated with stenosis of a major cerebral artery: Report of two cases.

Background: While hemodynamic stress can result in aneurysm formation, it rarely contributes to the development of peripheral aneurysms in collateral pathways. We report two patients with ruptured distal aneurysms in a collateral pathway associated with stenosis of a major cerebral artery.

Case Description: A 67-year-old man presented with intracerebral hemorrhage in the right frontal lobe.

Despite advancement of diagnostic and treatment modalities, subarachnoid hemorrhage (SAH) is still an entity of neurosurgical emergency with poor outcome. Recent reports indicated that hemodynamic stress might play an important role in rupture or the growth of cerebral aneurysms, but there is no consensus about how or which hemodynamic factor contribute to this phenomenon. In this report, magnetic resonance (MR)-based flow dynamics analysis was performed for a patient with SAH and the data obtained were directly compared with intraoperative findings.

Pilocytic astrocytoma with histological malignant features without previous radiation therapy--case report.

Pilocytic astrocytoma sometimes transforms to a malignant type, and previous radiation therapy is considered to be a key factor. We report a case of pilocytic astrocytoma with histological malignant features without previous radiation therapy. A 21-year-old man presented a sudden onset of severe headache.

Malignant peritoneal mesothelioma: quantitative analysis of asbestos burden.

Malignant mesotheliomas develop commonly in the pleural cavity and rarely arise in the peritoneal cavity. It is well established that asbestos exposure is related to malignant pleural mesothelioma, but the asbestos burden in the abdominal cavity in patients with malignant peritoneal mesothelioma has not been well studied. The purpose of the present study was therefore to report on an autopsy case of malignant peritoneal mesothelioma with quantitative analysis of the asbestos burden in tissues from the pleura and organs in the abdominal cavity.

Postpartum dissecting aneurysm of the posterior cerebral artery.

Intracranial dissecting aneurysm may cause subarachnoid hemorrhage (SAH) or infarction, and postpartum dissecting aneurysm is rare. A 30-year-old 6 days postpartum woman presented with posterior cerebral artery (PCA) dissection evolving dramatically over a short period. She had been well until 6 days after delivery when she suffered sudden onset of headache, vomiting, and unconsciousness.

Expression of Vicia villosa agglutinin (VVA)-binding glycoprotein in primary breast cancer cells in relation to lymphatic metastasis: is atypical MUC1 bearing Tn antigen a receptor of VVA?

Aberrant carbohydrate expression frequently occurs in breast cancer and may endow cells with metastatic potential. Here we first studied the relationship between expression of Vicia villosa agglutinin (lectin) (VVA)-binding carbohydrates and aggressive breast cancer. We then investigated the molecular characteristics of these glycoproteins and compared them with those of glycoproteins recognized by the mouse anti-Tn monoclonal antibody (MAb) HB-Tn1.

The structure of siglec-7 in complex with sialosides: leads for rational structure-based inhibitor design.

Siglecs (sialic acid binding Ig-like lectins) are transmembrane receptors for sialylated glycoconjugates that modulate cellular interactions and signalling events in the haematopoietic, immune and nervous systems. Siglec-7 is a structural prototype for the recently described family of immune inhibitory CD33-related siglecs and is predominantly expressed on natural killer cells and monocytes, as well as subsets of CD8 T-cells. Siglec-specific inhibitors are desired for the detection of masked and unmasked forms of siglecs, to aid in dissection of signalling pathways and as tools to investigate siglecs as potential therapeutic targets.

Lack of polymorphism in MUC1 tandem repeats in cancer cells is related to breast cancer progression in Japanese women.

MUC1 is a transmembrane molecule characterized by a repeated sequence of 20 amino acid (TAP PAHGVTSAPDTRPAPGS). Abnormal overexpression of MUC1 in cancer cells is thought to contribute to their aggressive growth, but molecular mechanisms associated with this effect are still unclear. Our current study aimed to clarify whether MUC1 expression as recognized by VU-3C6 anti-MUC1 mouse IgG monoclonal antibody (MAb) with a dominant epitope of 12 amino acids: GVTSAPDTRPAP, correlated with aggressive properties of human breast cancer.

[Dural arteriovenous fistula involving the cavernous sinus as the cause of intracerebral venous hemorrhage: a case report].

A case of cavernous sinus dural arteriovenous fistula (CS-DAVF) presenting intracerebral venous hemorrhage was reported. An 81-year-old woman was suffered from generalized convulsion accompanied by right conjunctival hyperemia and edema. Computed tomography scan revealed intracerebral hematoma involving right putamen and perifocal edema extraordinarily expanded to inferior part of the right frontal lobe and insular cortex.

Inhibition of DNA polymerases and DNA topoisomerase II by triterpenes produced by plant callus.

We found that some triterpene compounds could not only selectively inhibit the activities of mammalian DNA polymerase alpha (pol alpha) and beta (pol beta), but could also potently inhibit DNA topoisomerase II (topo II) [Biochem. J. 350 (2000) 757].

[A survival case of fluminant meningitis and multiple organ failure as overwhelming postsplenectomy infection (OPSI) syndrome].

A 39-year-old man, who had splenectomy following abdominal trauma 30 years previously, admitted to our hospital with high fever, headache and convulsion on January 8, 2002. Examination of cerebrospinal fluid (CSF) showed pleocytosis up to 10,000 cells/mm3 with polymorphonuclear cells dominant and elevated protein to 330 mg/dl and reduction of glucose to 15 mg/dl. Streptococcus pneumoniae was detected at the culture of CSF and blood.

Initial strength of highpressed extrusion poly-L-lactide screw.

We developed a poly-L-lactide material strengthened by a highpressed extrusion technique. The bending strength of a rod made of that material is higher than that of the same size rods made of poly-L-lactide strengthened by drawing technique, which has been used in clinical cases. The purposes of this study were, first to clarify if the initial strength of extrusion-strengthened poly-L-lactide screws is higher than that of draw-strengthened poly-L-lactide screws, and, secondly to investigate the safe torque for driving the screws in clinical usage.

Applications of vascular occlusion diminish disuse atrophy of knee extensor muscles.

Purpose: We have previously shown that the combination of low-intensity resistive exercise and moderate vascular occlusion induces in humans a marked increase in growth hormone secretion and muscular hypertrophy. The present study investigated the effects of vascular occlusion on the size of thigh muscles in patients who underwent an operation for the reconstruction of the anterior cruciate ligament to see whether it attenuates the disuse muscular atrophy without any exercise combined.

Methods: Two sessions of occlusive stimulus, each consisting of five repetitions of vascular occlusion (mean maximal pressure, 238 mm Hg) for 5 min and the release of occlusion for 3 min, were applied daily to the proximal end of the thigh from 3rd to 14th days after the operation.

Effects of resistance exercise combined with moderate vascular occlusion on muscular function in humans.

Acute and long-term effects of resistance exercise combined with vascular occlusion on muscular function were investigated. Changes in integrated electromyogram with respect to time (iEMG), vascular resistive index, and plasma lactate concentration were measured in five men either during or after elbow flexion exercises with the proximal end of the arm occluded at 0-100 mmHg. The mean iEMG, postexercise hyperemia, and plasma lactate concentration were all elevated with the increase in occlusion pressure at a low-intensity exercise, whereas they were unchanged with the increase in occlusion pressure at high-intensity exercise.

Reduced expression of E-cadherin in oral squamous cell carcinoma: relationship with DNA methylation of 5' CpG island.

E-cadherin, a Ca2+-dependent cell-cell adhesion molecule, is involved in the maintenance of the epithelial phenotype. The reduction of its expression is considered to be important in the invasive and metastatic potential of carcinomas. A series of 52 primary oral squamous-cell carcinomas (SCCs) were studied immunohistochemically for E-cadherin expression in microwave-treated paraffin-embedded sections using a monoclonal antibody (HECD-1).

Chronic exertional compartment syndrome in lower legs: localization and follow-up with thallium-201 SPECT imaging.

Unlabelled: The purpose of this study was to ascertain whether 201Tl SPECT imaging of the leg is useful in precise localization of the ischemic compartment involved in chronic exertional compartment syndrome (CECS).

Methods: Imaging and quantitative analyses of postexercise 201Tl SPECT leg examinations were retrospectively performed in nine patients with clinically diagnosed CECS and eight control subjects. Imaging and quantitative criteria for the ischemic compartment were decreased 201Tl perfusion less than the lower limits of normal, which were defined as 2 s.

[Association between expression of integrin (VLA-3, VLA-5) and malignancy in human colon-cancer].

The expression of the VLA-integrins beta 1, -2, -3, -5, -6 and alpha v beta 3 was studied immunohistochemically in the tissue sample from human colon-cancer. Furthermore, histological stage-grouping of the colon-cancer (I-V), additional vascular invasion such as lymphatic and blood-vessel invasion, was made to express the malignancy of the carcinoma, for making a comparison with the expression of the integrins. Immunohistologically, beta 1, VLA-2, and alpha v beta 3 were mildly expressed in the carcinoma cells and VLA-6, markedly expressed.

Correlation between angiographic features and pathologic changes in breast carcinoma.

The correlation between angiographic tumor blushing and underlying pathologic factors was studied. Twenty female patients with breast cancer were divided into three groups according to the degree of tumor blushing. Primary tumors were divided into three areas: marginal, central and intermediate areas.
