[A contribution of the hospital pharmacy for home parenteral nutrition--utilization of a HPN report form].

Home parenteral nutrition (HPN) is a useful measure when terminal cancer patients are hoping to have a transition from the hospital to a home medical care arrangement. Our department has been supporting their family by mixing injections at our clean room. We created a HPN report form to check for an incompatibility of injections, and to prepare an infusion set by our department.
