Article Synopsis
  • Patients with familial adenomatous polyposis (FAP) face both medical risks and social challenges, particularly related to marriage and childbirth after undergoing colorectal surgery.
  • A study of 161 Japanese patients showed that the overall marriage rate post-surgery (57.8%) was similar to that of the general Japanese population, but those unmarried before surgery had a low marriage rate (16.0%).
  • Factors like younger age and genetic testing significantly increased the likelihood of getting married post-surgery, and 71% of those who married after surgery went on to have successful pregnancies.

Article Synopsis
  • Patients with familial adenomatous polyposis (FAP) are at a higher risk of gastric neoplasms, including adenomas and cancer, and this study investigates their incidence over time.
  • A cohort of 443 FAP patients was analyzed, revealing that by age 50, 22.8% had gastric adenomas and 7.6% had gastric cancer, with the highest risk for adenomas occurring around age 65.
  • The study suggests that regular monitoring of the upper gastrointestinal tract in older FAP patients could improve early detection of gastric cancer, indicating a need for more frequent check-ups as patients age.

Article Synopsis
  • * In a study of 451 FAP patients, 59% had duodenal polyps and 18% had ampullary polyps; duodenal cancer incidence was 4.7% and ampullary cancer was 18% in those with respective polyps.
  • * The study suggests that patients with duodenal polyps require regular endoscopic monitoring, especially those with intermediate- or profuse-type APC variants, due to the high rate of progression of these polyps.*

Article Synopsis
  • Understanding the branch patterns of internal iliac vessels, particularly veins, is crucial for safely performing minimally-invasive pelvic surgeries.
  • A study of 30 patients examined the anatomical variations of these vessels using surgical videos and 3D simulations, focusing on several key arteries and veins.
  • Results showed a dominant branch pattern for the internal iliac vein, with high correlation between the arteries and veins, which can improve surgical outcomes and safety in procedures.

Introduction: In Japanese patients with familial adenomatous polyposis (FAP), colectomy tends to be postponed or avoided.

Aim: This study aimed to clarify the current clinical practice from a Japanese multicenter cohort study database.

Methods: We analyzed the records of 250 patients with non-dense FAP who did not require colorectal cancer removal.

A 55-year-old man was referred for surgery after colonoscopy revealed type 3 advanced lower rectal cancer in the lower rectum. CT and MRI scan showed no distant metastasis but on the left side of the rectum, there was a 34×30 mm large mass suspicious of lymph node metastasis, which had left-sided wall pelvic fascia invasion. We performed preoperative chemoradiotherapy(CRT)to ensure a secure surgical margin.

Circulating microRNAs (miRNAs) are considered promising biomarkers for diagnosis, prognosis, and treatment efficacy of diseases. However, usefulness of circulating miRNAs as biomarkers for hereditary gastrointestinal diseases have not been confirmed yet. We explored circulating miRNAs specific for patients with familial adenomatous polyposis (FAP) as a representative hereditary gastrointestinal disease.

Purpose: Data on long-term outcomes of familial adenomatous polyposis (FAP) are unclear in Japan because a nationwide registry system is lacking. We assessed overall survival, incidence of neoplasms, fecal incontinence, and postoperative follow-up status of patients with FAP treated surgically in our hospital.

Methods: In total, 154 patients with FAP who underwent radical surgery from 1981 to 2017 in our department were available for the questionnaire.

The first asymmetric total synthesis of (-)-ophiodilactone A and (-)-ophiodilactone B, isolated from the ophiuroid (Ophiocoma scolopendrina), is reported. The key features of the synthesis include the highly stereocontrolled construction of the structurally congested γ-lactone/δ-lactone skeleton through an asymmetric epoxidation, diastereoselective iodolactonization, and intramolecular epoxide-opening with a carboxylic acid, and biomimetic radical cyclization of ophiodilactone A to ophiodilactone B.

NW-G01, a cyclic hexapeptide antibiotic, and 34-epi-NW-G01 were synthesized by the highly stereoselective convergent approach for the first time, thereby unambiguously determining the absolute structure of NW-G01.
