Article Synopsis
  • * Bone marrow tests revealed he had myelodysplastic syndrome/myeloproliferative neoplasm, which later transitioned into acute myeloid leukemia six months later, despite a temporary improvement after stopping tocilizumab.
  • * After receiving unrelated bone marrow stem cell transplantation and chemotherapy, he achieved complete remission, with both his leukemia and joint pain improving post-treatment, highlighting the risks of malignancies in JIA patients on immunosuppressive therapy.

: The respiratory adjusted shock index (RASI) is a risk score whose usefulness in patients with sepsis and trauma has previously been reported. However, its relevance in elderly emergency patients with medical diseases is yet to be clarified. This study assessed the usefulness of the RASI, which can be evaluated without requiring special equipment, to provide objective and rapid emergency responses.

Article Synopsis
  • This study investigates how pre-existing anti-PEG antibodies affect the immune response to the anti-SARS-CoV-2 mRNA vaccine in patients with hematological and autoimmune diseases.
  • Findings indicate that antibody levels were generally higher in autoimmune patients, but baseline anti-PEG antibodies did not correlate with anti-SARS-CoV-2 antibodies in hematological patients.
  • The research concludes that while age and treatment with anti-CD20 negatively affected antibody response in hematological patients, higher anti-PEG antibody levels were linked to better SARS-CoV-2 antibody responses in autoimmune patients.

Background: Although the palliative prognostic index (PPI), objective prognostic score (OPS), and neutrophil-lymphocyte ratio/albumin ratio (NLR/Alb) are well-known prognostic indicators for cancer patients, they do not provide clarity when it comes to predicting prognosis in patients without cancer who receive home-visit palliative care.

Objective: The aim of this study was to determine whether PPI, OPS, and NLR/Alb can predict prognosis for patients without cancer who received home-visit palliative care.

Design: This is a retrospective study.

Since diabetes and its complications have been thought to exaggerate cardiorenal disease, resulting in a short lifespan, we investigated causes of death and lifespans in individuals with and without diabetes at a Japanese community general hospital during the period from 2011 to 2020. Causes of death and age of death in individuals with and those without diabetes were compared, and associations between medications used and age of death were statistically analyzed. A total of 2326 deaths were recorded during the 10-year period.

It has previously been unclear whether the accumulation of advanced glycation end products, which can be measured using skin autofluorescence (SAF), has a significant role in diabetic kidney disease (DKD), including glomerular injury and tubular injury. This study was therefore carried out to determine whether SAF correlates with the progression of DKD in people with type 2 diabetes (T2D). In 350 Japanese people with T2D, SAF values were measured using an AGE Reader, and both urine albumin-to-creatinine ratio (uACR), as a biomarker of glomerular injury, and urine liver-type fatty acid-binding protein (uLFABP)-to-creatinine ratio (uL-FABPCR), as a biomarker of tubular injury, were estimated as indices of the severity of DKD.

Dehydroepiandrosterone sulfate (DHEAS) is thought to be associated with life expectancy and anti-aging. Although skeletal muscle disorders are often found in diabetic people, the clinical significance of DHEAS in skeletal muscle remains unclear. Therefore, we aimed to determine whether DHEAS is associated with the development of skeletal muscle disorders in individuals with type 2 diabetes (T2D).

A 55-year-old male was referred to our hospital after complaining of a sore throat for a month. Physical examination revealed a disturbance in consciousness, nuchal rigidity, painful multiple ulcers in the oral cavity, and erythema, the size of rice grains on the body. Hematological examination showed the following results: white blood cells, 7,910/µl (abnormal lymphocytes 2%), LDH, 203 U/l, corrected calcium, 11.

Background: Anemia is one of the common complications of diabetes and is associated with mortality. Phase angle (PhA), ratio of extracellular water to total body water (ECW/TBW) and skeletal muscle mass index (SMI) estimated by bioelectrical impedance analysis (BIA) have been used as prognostic indicators for various chronic diseases and frailty. We aimed to clarify the clinical significance of PhA, ECW/TBW and SMI for anemia in patients with diabetes.

The prognosis of relapsed or refractory lymphoma with central nervous system (CNS) involvement remains poor because of the lack of anticancer drugs with sufficient CNS penetration. [Case 1] A 65-year-old man was diagnosed with Stage IV mantle cell lymphoma. After two courses of chemotherapy and autologous hematopoietic stem cell (HSC) collection, urinary retention with fever developed.

Article Synopsis
  • * The examination revealed he had acute pancreatitis, with swollen pancreas and dilated pancreatic duct, which resembled autoimmune pancreatitis but was complicated by acute myeloid leukemia.
  • * Chemotherapy led to remission of both leukemia and pancreatitis, suggesting that leukemia cell invasion may have caused the pancreatitis, highlighting the need to consider leukemia when diagnosing acute pancreatitis.

Synopsis of recent research by authors named "Taiki Hori"

  • - Taiki Hori's research primarily focuses on the interactions between various medical conditions, treatments, and immune responses, especially in vulnerable populations like those with hematological disorders and the elderly.
  • - Recent findings highlight significant associations such as the progression of acute myeloid leukemia during immunosuppressive therapy for juvenile idiopathic arthritis and the impact of the respiratory adjusted shock index as a predictor of outcomes in elderly patients.
  • - Other notable contributions include exploring the effects of anti-SARS-CoV-2 vaccines on immune responses in patients with pre-existing conditions, and assessing prognostic indicators in non-cancer palliative care, thereby aligning his work with advancing understanding for patient stratification in clinical settings.
