Background: With the escalating utilization of online health education programs and the growing prevalence of infertility, there is an urgent requirement for a comprehensive and effective online platform that caters to the unique needs of couples facing infertility. Considering invaluable functions of medical record department - as legal document and continuity of health care - "failure mode and effects analysis (FMEA)" utilized to identify the ways a process can fail, and how it can be made safer.
