Article Synopsis
  • This study aimed to investigate the demographics, clinical characteristics, and outcomes of patients with postpartum hemorrhage (PPH) who underwent transcatheter arterial embolization (TAE) in Japan from 2012 to 2020.
  • Out of 64,893 PPH patients, only 2,705 (4.2%) had TAE, with uterine atony being the leading cause; a significant portion required hysterectomies, and some surgeries occurred more than a week after TAE.
  • The findings emphasize that while TAE can control bleeding, clinicians should be aware that hysterectomies may still be necessary later, highlighting the importance of fertility-preserving treatment options for PPH patients.

Article Synopsis
  • - The study examines how the timing of resuming direct oral anticoagulants (DOACs) after colorectal endoscopic submucosal dissection (ESD) affects patient outcomes, finding inconsistent guidelines due to a lack of large studies.
  • - Researchers analyzed a large dataset from Japan, focusing on patients who resumed DOACs either the day after or 2-3 days following ESD and assessed risks of delayed bleeding and thromboembolic events.
  • - Findings show that early resumption of DOACs did not significantly increase the risk of delayed bleeding but notably reduced thromboembolic events, suggesting it may be safer to restart DOACs sooner after the procedure.

It is expected but unknown whether machine-learning models can outperform regression models, such as a logistic regression (LR) model, especially when the number and types of predictor variables increase in electronic health records (EHRs). We aimed to compare the predictive performance of gradient-boosted decision tree (GBDT), random forest (RF), deep neural network (DNN), and LR with the least absolute shrinkage and selection operator (LR-LASSO) for unplanned readmission. We used EHRs of patients discharged alive from 38 hospitals in 2015-2017 for derivation and in 2018 for validation, including basic characteristics, diagnosis, surgery, procedure, and drug codes, and blood-test results.

  • This study investigates the effectiveness of drainage fluid amylase (DFA) in predicting postoperative pancreatic fistula (CR-POPF) rates after two types of surgery: open distal pancreatectomy (ODP) and laparoscopic distal pancreatectomy (LDP).
  • Researchers reviewed data from 294 patients and found that the rate of CR-POPF was significantly higher after ODP compared to LDP, with DFA levels on postoperative days one and three showing distinct optimal cutoff values for the two procedures.
  • The findings indicate that while DFA on day three can predict CR-POPF for both surgeries, the cutoff value is notably higher for patients who underwent LDP compared to those who had ODP, suggesting different thresholds may

Background: Esophageal variceal bleeding is a severe complication associated with liver cirrhosis and typically necessitates endoscopic hemostasis. The current standard treatment is endoscopic variceal ligation (EVL), and Western guidelines recommend antibiotic prophylaxis following hemostasis. However, given the improvements in prognosis for variceal bleeding due to advancements in the management of bleeding and treatments of liver cirrhosis and the global concerns regarding the emergence of multidrug-resistant bacteria, there is a need to reassess the use of routine antibiotic prophylaxis after hemostasis.
