Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis. Half of the cases are associated with an immune dysfunction and are frequently triggered by pathergy such as a tissular aggression via surgery or burn wounds. A patient with ulcerative colitis presented a PG at the site of an iontophoresis patch for tendinopathy.

This report describes an unusual case of wide tissue coverage by «all around» flap by Mac Gregor technique in a victim of an extensive burn of the left upper limb which occurred on 08/08/1997 and was responsible for a progressive flessum of the wrist and a hyperextension of the fingers with a complete loss of their mobility. The intervention is done in one step with a wide release of the scarred areas of the wrist and hand and the realization of the free inguinal flap in «all around».

However, first responders are not typically trained to conduct sonographic exams and/or do not have access to an expensive ultrasound device. Therefore, for successful deployment of ONSD measurement in-field, we believe that first responders must have access to low-cost, portable ultrasound and be assisted by artificial intelligence (AI) systems that can automatically interpret the optic nerve sheath ultrasound scan.
