Parathyroid cancer (PC) is extremely resistant to chemotherapy and radiotherapy (RT), but hormonally functional by producing excessive parathyroid hormone (PTH), causing remarkable hypercalcemia even in biochemical disease recurrence. Accordingly, management of hypercalcemia by calcimimetics and bisphosphonates has been main treatment for unresectable PC. Here, we report a case of unresectable tumor mutational burden (TMB)-high recurrent PC that has been effectively controlled by pembrolizumab (PEM) with RT.

Article Synopsis
  • Flexible laryngoscopy (FL) is the standard method for diagnosing vocal cord paralysis (VCP), but it can be uncomfortable for patients; dynamic digital radiography (DDR) offers a less invasive alternative with better ease of use.
  • A study involving 78 patients showed that DDR had 67% sensitivity and 100% specificity for diagnosing VCP, with a critical vocal cord movement (VCM) cutoff of 2.4 mm.
  • DDR showed promise for diagnosing postoperative VCP and predicting which patients might develop permanent VCP, with impressive results regarding the detection of transient VCP.

Background: Acupuncture is known to improve exercise capacity in patients with chronic obstructive pulmonary disease (COPD), but its mechanism remains unknown. Whether acupuncture improves exercise capacity in patients with COPD through alleviation of leg fatigue and dyspnea is examined by applying causal mediation analysis to previous trial data.

Methods: Sixty-two patients with COPD completed treatments with either real or placebo acupuncture once a week for 12 weeks.
