Background: Observational data have suggested that patients with moderate to severe ischemia benefit from revascularization. However, this was not confirmed in a large, randomized trial.
Objectives: Using a contemporary, multicenter registry, the authors evaluated differences in the association between quantitative ischemia, revascularization, and outcomes across important subgroups.
Stress radionuclide myocardial perfusion imaging (MPI) has been well-established as a useful modality for assessing the status of the coronary circulation in post-coronary artery bypass graft (CABG) patients. CABG by itself escalates progression of atherosclerosis or thrombosis in bypassed native coronary arteries. In most cases MPI will be employed in post-CABG patients who are experiencing symptoms.

Stress image acquisition with rubidium-82 (Rb) PET occurs at peak stress compared to 30-60 minutes post-stress with SPECT. We aimed to evaluate the prognostic value of TID in patients undergoing Rb PET MPI.

We present the design and initial results from the updated registry. The updated REFINE SPECT is a multicenter, international registry with clinical data and image files.

The ability to adequately augment blood flow, measured by myocardial blood flow reserve (MBFR), is associated with lower major adverse cardiovascular events and all-cause mortality. The age-specific ranges of MBFR in patients without demonstrable coronary artery disease have not been well established.
