In this case report, we have described a novel variant of mirror-image dextrocardia associated with viscero-atrial situs solitus, and not inversus, and the embryological basis for this presentation. The use of a pericardial baffle to correct the underlying intracardiac defects and the importance of confirming the anteroposterior relations of the atria, and not just their left-right relations, while deciphering the anatomy of dextrocardia.
