Background: Patellofemoral joint (PFJ) issues after total knee arthroplasty (TKA) are becoming a topic of interest once again. Due to the complex three-dimensional shape of the trochlea, various two-dimensional proxy measurements on plain X-rays have been described. One of these measurements is the anterior femoral offset (AFO).

It has 12 questions, with each question having the same weight for the overall score. Some authors have observed a significant ceiling effect, especially when distinguishing slight postoperative differences.

Guideline-based surgical cartilage therapy for focal cartilage damage offers highly effective possibilities to sustainably reduce patients' complaints and to prevent or at least delay the development of early osteoarthritis. In the knee joint, it has the potential to reduce almost a quarter of the arthroses requiring joint replacement caused by cartilage damage. Biologically effective injection therapies could further improve these results.

Article Synopsis
  • Total knee arthroplasties (TKAs) in patients aged ≤35 years are uncommon but essential for conditions like juvenile idiopathic arthritis and osteoarthritis; however, few studies investigate their long-term outcomes.
  • A study of 185 TKAs from 119 young patients showed a decline in implant survivorship from 84% at 5 years to 37% at 20 years, with common revision reasons being aseptic loosening and infection.
  • Despite lower survivorship rates, a majority of surveyed patients reported significant pain relief and functional improvement after 17 years post-surgery, indicating that TKAs can be beneficial for this age group despite higher revision risks associated with older age and prosthesis types.
