In vivo demonstration of an antithrombin effect of abciximab.

Abciximab prolonged the activated clotting time (ACT) in a post hoc analysis from the Evaluation of IIb/IIIa Platelet Receptor Antagonist 7E3 in Preventing Ischemic Complications trial and an in vitro study has suggested an antithrombin effect of platelet glycoprotein IIb/IIIa inhibition. The purpose of this study was to evaluate the in vivo effects of abciximab on ACT and thrombin generation. In 46 patients undergoing coronary intervention, 24 received heparin and abciximab (group I), whereas 22 received heparin alone (group II).

Microvascular cytomegalovirus endothelialitis of the lung: a possible cause of secondary pulmonary hypertension in a patient with AIDS.

Postmortem examination of the lungs of a patient with advanced AIDS who had developed pulmonary arterial hypertension late in the course of the illness demonstrated extensive cytomegalovirus (CMV) infection in endothelial cells of the lung microvasculature. Enlarged CMV-infected endothelial cells were present in virtually all histologic sections of the lungs, protruded into and compromised the lumens of the small vessels they lined, and were estimated by image cytometry of immunohistochemically stained sections to comprise 0.8% of the total lung tissue volume.

Incidence of subclavian vein thrombosis detected during first-pass phase of radionuclide angiocardiogram.

A 79-yr-old woman with asymptomatic subclavian vein thrombosis associated with transvenous pacemaker electrode and congestive heart failure is reported. The subclavian thrombosis was discovered accidentally from the first-pass radionuclide angiogram that is routinely performed with the intravenous bolus injection of the radiopharmaceutical for a gated blood-pool study. It demonstrated venous obstruction at the level of the subclavian vein and abnormal collateral circulation over the chest wall consistent with subclavian thrombosis.

Efficacy of percutaneous transluminal coronary angioplasty compared with single-vessel bypass.

The use of percutaneous transluminal coronary angioplasty has been increasing rapidly. When the procedure is successful, the clinical relief of symptoms is similar to that achieved with direct coronary artery bypass. It has been suggested that the angioplasty procedure, however, can accomplish these results with potentially less morbidity and mortality, along with a shorter hospital stay.

Angiographic demonstration of coronary sinus thrombosis: a potential consequence of trauma to the coronary sinus.

Abnormalities of the coronary sinus are rarely encountered. A case is presented demonstrating for the first time the angiographic appearance of coronary sinus thrombosis. This may have been the result of surgical trauma during mitral valve replacement or inadvertent cannulation of the coronary sinus during right heart catheterization or pacemaker insertion.
