endophthalmitis is a rare cause of endogenous endophthalmitis, with very few cases documented in the US. We present a male patient in his 60s with a history of latent tuberculosis who presented to the hospital with complaints of acute bilateral vision loss that began three days prior to admission. The workup revealed bacteremia, a large hepatic abscess, severe orbital swelling, and acute angle-closure glaucoma.

Article Synopsis
  • Left heart disease (LHD) is a leading cause of pulmonary hypertension (PH), further classified into isolated post-capillary (ipcPH) or combined pre- and post-capillary PH (cpcPH).
  • Recent clinical trials emphasize avoiding pulmonary arterial hypertension treatments for PH-LHD patients outside of trials, as these treatments may cause harm.
  • A new PH-LHD staging system categorizes patients into four stages based on their clinical and hemodynamic characteristics, which aims to improve diagnosis and treatment through a more individualized approach.

Article Synopsis
  • Sleep architecture plays a crucial role in understanding sleep quality and hyperarousal in individuals with insomnia compared to those without insomnia.
  • A study analyzed data from various clinical trials and studies, assessing sleep patterns in 1710 individuals with insomnia and 1455 without, using advanced techniques including machine learning.
  • Results revealed that those with insomnia experienced higher wakefulness from all sleep stages, greater alpha and theta activity during wakefulness, and marked changes in sleep spindle characteristics, indicating a persistent state of hyperarousal affecting sleep quality.

Targeting oncogene addictions have changed the history of subsets of malignancies and continues to represent an excellent therapeutic opportunity. Nonetheless, alternative strategies are required to treat malignancies driven by undruggable oncogenes or loss of tumor suppressor genes and to overcome drug resistance also occurring in cancers addicted to actionable drivers. The discovery of non-oncogene addiction (NOA) uncovered novel therapeutically exploitable "Achilles' heels".

Study Objectives: Post hoc analysis to evaluate the effect of daridorexant on sleep architecture in people with insomnia, focusing on features associated with hyperarousal.

Methods: We studied sleep architecture in adults with chronic insomnia disorder from two randomized phase 3 clinical studies (Clinicaltrials.gov: NCT03545191 and NCT03575104) investigating 3 months of daridorexant treatment (placebo, daridorexant 25 mg, daridorexant 50 mg).
