Background: Hypertension therapy in older adults is often suboptimal, in part because of inadequate suppression of the renin-angiotensin-aldosterone system (RAAS). We hypothesised that distinct endotypes of RAAS activation before noncardiac surgery are associated with increased risk of myocardial injury.

Methods: This was a prespecified exploratory analysis of a multicentre randomised controlled trial (ISRCTN17251494) which randomised patients ≥60 yr old undergoing elective noncardiac surgery to either continue or stop RAAS inhibitors (determined by pharmacokinetic profiles).

Article Synopsis
  • - PIONEER is a European initiative that combines the efforts of 34 organizations across 9 countries to enhance prostate cancer screening, diagnosis, and treatment using big data analytics.
  • - The project has created a comprehensive platform that integrates various data sources, helping to respond to critical research questions and improve understanding of prostate cancer management at different stages.
  • - Despite successes, challenges with real-world data persist, necessitating ongoing collaboration and knowledge sharing, which PIONEER aims to support as part of the UroEvidenceHub initiative by the European Association of Urology.

Background: The average number of times a person will have surgery in their lifetime, and the amount of surgical healthcare resources they use, is unknown. Lifetime risk is a measure of the risk of an average person having a specific event within their lifetime. We report the lifetime risk of surgery and the change observed during the first year of the COVID-19 pandemic.

Epitaxial crystallization of complex oxides provides the means to create materials with precisely selected composition, strain, and orientation, thereby controlling their functionalities. Extending this control to nanoscale three-dimensional geometries can be accomplished via a three-dimensional analog of oxide solid-phase epitaxy, lateral epitaxial crystallization. The orientation of crystals within laterally crystallized SrTiO systematically changes from the orientation of the SrTiO substrate.

Article Synopsis
  • Metastatic prostate cancer (PCa) makes up about 5% of new diagnoses in Western countries, highlighting the need for effective systemic therapies, but concerns exist regarding the real-world applicability of randomized controlled trials (RCTs) used to establish treatment protocols.
  • The PIONEER project, part of the Innovative Medicine Initiative, aims to enhance cancer care in Europe by focusing on improving health outcomes and the efficiency of health systems through better understanding of RCT findings in real-world settings.
  • A systematic review will compare characteristics of patients from RCTs with those in population-based databases to assess how well RCT results apply to actual patients with newly diagnosed metastatic PCa and determine how many real-world patients qualify for RCT participation.*
