Position of Cladribine Tablets in the Management of Relapsing-Remitting Multiple Sclerosis: An Expert Narrative Review From the United Arab Emirates.

The use of immune reconstitution therapies (IRT) in patients with relapsing-remitting multiple sclerosis (RRMS) is associated with a prolonged period of freedom from relapses in the absence of continuously applied therapy. Cladribine tablets is a disease-modifying treatment (DMT) indicated for highly active relapsing multiple sclerosis (MS) as defined by clinical or imaging features. Treatment with cladribine tablets is effective and well tolerated in patients with active MS disease and have a low burden of monitoring during and following treatment.

The number of disease-modifying treatments (DMDs) for relapsing-remitting multiple sclerosis has increased. DMDs differ not only in their efficacy and safety/tolerability, but also in the treatment burden of, associated with their initiation, route/frequency of administration, maintenance treatment and monitoring. High-efficacy DMDs bring the prospect of improved suppression of relapses and progression of disability, but may have serious safety issues, and burdensome long-term monitoring.

Ischaemic stroke as the first presentation of occult squamous cell cancer.

A 39 years old female presented with sudden onset of left sided weakness and CT scan brain confirmed an ischaemic stroke. Extensive investigations looking for the underlying cause were un-rewarding. She presented few days later with confusion and fever and was found to have multiple new cerebral infarcts, disseminated intravascular coagulation, rectal mass and liver metastases.

Intracranial tuberculoma.

Six patients with cerebral tuberculoma seen over a 2 year period are reported. Their clinical presentations, the result of investigations including radiological findings and their response to medical treatment are discussed and correlated with other reports in the literature. Eighty five percent of the patients were above the age of 30 years.

An unusual association between oral sarcoma and bulbar palsy.

A 90 year old male presented with symptoms and signs of right lower cranial nerves palsy. A small mass was found on the right side at the back of his mouth. Light microscopy and histoimmunochemical studies of the biopsy of the mass showed an alveolar soft part sarcoma arising from the right myohyoid muscle.
