Introduction: Permanent hypoparathyroidism (pHypoPT) is the most common permanent complication of total thyroidectomy. We aim to describe the incidence and predictors of hypoparathyroidism in a consecutive series of patients treated in a high-volume centre and define strategies to reduce the risk of pHypoPT.

Methods: 1182 patients who underwent total thyroidectomy between April 2018 and June 2022 were analyzed.

Background: With the current shift toward de-escalation of surgical management in low-risk papillary thyroid cancer (PTC), understanding predictors and the clinical significance of additional tumors in the contralateral lobe is important. This study investigated the histopathologic predictors of bilateral disease in low-risk PTC patients and the utility of preoperative ultrasonography in guiding completion thyroidectomy decisions.

Methods: Patients treated with total thyroidectomy (TT) for low-risk PTCs (< 4 cm) at the Endocrine Surgical Unit of the Royal North Shore Hospital, University of Sydney from 2013 to 2020 were identified from a prospectively maintained database.

Background: Thyroid rests are common and arise during embryological development, with aberrant descent of thyroid tissue along the thyro-thymic tract. These give rise to a range of pathologies, including goitre formation and malignancy. Thyroid rests have been graded I-IV based on their connection to the thyroid gland proper - grade I is a short protuberance, grade II & III are connected via a pedicle and fibrous band respectively, and grade IV is completely disconnected.

Article Synopsis
  • This study assesses the outcomes of adrenalectomy for pheochromocytoma in obese versus nonobese patients, using data from 46 centers from 2012 to 2022.
  • It finds that while obesity does not significantly increase the rate of complications or comprehensive complication index (CCI), it does lead to a longer hospital stay for patients.
  • Furthermore, minimally invasive surgical techniques, like laparoscopic and robotic surgeries, are associated with less morbidity in obese patients, and a retroperitoneal approach may provide additional benefits.

Background: Lymph node yield (LNY) is a validated quality control parameter in colorectal cancer surgery, with >12 nodes reflecting an adequate oncological resection. No formal guidelines exist in the context of central and lateral compartment lymph node dissection for papillary thyroid cancer (PTC). This study aimed to investigate the association between LNY and regional recurrence in PTC patients, and to define a threshold LNY that indicates adequate compartmental lymphadenectomy.

Background: Currently pathways to practice in endocrine surgery vary based on location and surgical training programme. International data highlights the impact of surgeon volume on outcomes, and the importance of understanding the learning curve in developing minimum training competencies. This study aims to explore how surgeons obtain competence in endocrine surgery in Australia and New Zealand, and perceptions around competence and scope of practice.

Article Synopsis
  • The study explores the use of an online clinical support tool designed to minimize judgment variability, or "noise," in prescribing radioactive iodine (RAI) for patients with differentiated thyroid cancer (DTC).
  • It involved applying the tool to 103 patients treated from 2021-2022 and comparing it to 393 patients treated from 2017-2021, showing significant reductions in RAI dosages for intermediate and high-risk patients.
  • The results indicate that the tool effectively reduces variability in treatment decisions without increasing recurrence rates, suggesting it's a beneficial resource for clinical practice in DTC management.

Anaplastic thyroid cancer (ATC), a rare and highly aggressive malignancy, is characterized by an exceptionally poor prognosis, where the majority of patients present with extensive local invasion and/or distant metastases. 20-30% of ATCs harbor the BRAF-V600E mutation. Neoadjuvant BRAF-targeted therapy may have the potential to downstage and facilitate surgical resection for patients with locally advanced and unresectable primary tumors with BRAF mutation and may convey a survival advantage in those with metastatic disease.

Background: Hashimoto's thyroiditis (HT) is managed with thyroid hormone replacement to maintain a euthyroid state. A subset of patients have refractory symptoms, which improve with thyroidectomy (TT). There remains a reluctance to proceed with surgery due to perceptions of complications, and limited data availability regarding improvements in quality of life (QoL).

Article Synopsis
  • Anaplastic thyroid carcinoma is one of the deadliest cancers and frequently occurs alongside differentiated thyroid cancers, but its aggressive nature is not well understood.
  • Researchers analyzed tumor DNA from 329 regions of thyroid cancer, revealing that anaplastic thyroid carcinomas have more mutations and unique mutational patterns compared to other thyroid cancers.
  • The study shows that although anaplastic and differentiated thyroid carcinomas can occur in the same patient, they have different mutated genes and share a common genetic origin that allows them to develop from a similar malignant environment.

Aims: Recently, there have been attempts to improve prognostication and therefore better guide treatment for patients with medullary thyroid carcinoma (MTC). In 2022, the International MTC Grading System (IMTCGS) was developed and validated using a multi-institutional cohort of 327 patients. The aim of the current study was to build upon the findings of the IMTCGS to develop and validate a prognostic nomogram to predict recurrence-free survival (RFS) in MTC.

Article Synopsis
  • Somatostatin receptor imaging using Ga-DOTATATE PET/CT enhances the diagnostic capabilities for medullary thyroid cancer (MTC), allowing for better understanding of tumor characteristics.
  • In a study of 103 MTC patients, only 41% showed positive SST expression, but SST positivity did not correlate with survival benefits or imaging results.
  • The findings suggest that the Total Lesion Activity (TLA) from Ga-DOTATATE PET/CT could help predict patient survival, while combining it with F-FDG-PET/CT may provide a more accurate assessment of metastatic disease.

Purpose: Multifocal disease in PTC is associated with an increased recurrence rate. Multifocal disease (MD) is underdiagnosed with the current gold standard of pre-operative ultrasound staging. Here, we evaluate the use of EMI-137 targeted molecular fluorescence-guided imaging (MFGI) and spectroscopy as a tool for the intra-operative detection of uni- and multifocal papillary thyroid cancer (PTC) aiming to improve disease staging and treatment selection.

Article Synopsis
  • Tall cell papillary thyroid carcinoma (TC-PTC) is debated in terms of its independent prognostic value due to varying criteria and its link to other adverse features.
  • A study analyzed 487 PTCs and found that the percentage of tall cells significantly impacted disease-free survival (DFS), particularly showing worse outcomes with higher tall cell proportions.
  • The findings suggest that TC-PTC's clinical significance varies based on the context, being particularly crucial for tumors classified as low risk by American Thyroid Association guidelines.

Background: The 2015 American Thyroid Association (ATA) Guidelines permit thyroid lobectomy (TL) or total thyroidectomy in the management of low-risk papillary thyroid cancer (PTC). As definitive risk-stratification is only possible post-operatively, some patients may require completion thyroidectomy (CT) after final histopathological analysis.

Methods: A retrospective cohort study of patients undergoing surgery for low-risk PTC in a tertiary referral centre was undertaken.
