Article Synopsis
  • A study was conducted to analyze how often dermatologic surgeons face challenges in identifying the locations of skin cancer tumors before surgery.
  • Out of over 17,000 cases, only 0.60% involved uncertain sites, with these cases more common in older, male patients who had biopsies done over a month prior.
  • Surgeons commonly used various methods to confirm tumor locations, achieving success in identifying 82% of these uncertain sites, while some surgeries were delayed, especially for non-facial tumors.

Tumors with pathogenic DICER1 mutation are rare and encompass sporadic or hereditary benign, intermediate and malignant tumors. DICER1-associated sarcomas are heterogeneous; however, the prototypical ones in the GYN-tract include embryonal rhabdomyosarcoma, adenosarcoma and moderately to poorly differentiated Sertoli-Leydig tumor. In this report, we present three unique uterine sarcomas with DICER1 mutation and remarkable diffuse round/spindle cell morphology.

Somatic mosaic variants contribute to focal epilepsy, but genetic analysis has been limited to patients with drug-resistant epilepsy (DRE) who undergo surgical resection, as the variants are mainly brain-limited. Stereoelectroencephalography (sEEG) has become part of the evaluation for many patients with focal DRE, and sEEG electrodes provide a potential source of small amounts of brain-derived DNA. We aimed to identify, validate, and assess the distribution of potentially clinically relevant mosaic variants in DNA extracted from trace brain tissue on individual sEEG electrodes.

  • Ravidasvir (RDV) is a new drug that effectively inhibits the hepatitis C virus (HCV) replication and has shown promising results when combined with Sofosbuvir (SOF) in trials involving adults, including those co-infected with HIV.
  • In a study conducted in Thailand and Malaysia, RDV was given to both patients with and without cirrhosis for various durations, and pharmacokinetics (how the drug behaves in the body) were evaluated through blood sampling at different intervals.
  • The analysis revealed that RDV levels were influenced by several factors; however, these variations were not significant enough to impact the drug's effectiveness across the diverse group of participants, and there were no significant drug interactions for

Historical demographic research shows that the factors influencing mortality risk are labile across time and space. This is particularly true for datasets that span societal transitions. Here, we seek to understand how marriage, migration, and the local economy influenced mortality dynamics in a rapidly changing environment characterized by high in-migration and male-biased sex ratios.

Objectives: Self-assessment of perceived communication difficulty has been used in clinical and research practices for decades. Such questionnaires routinely assess the perceived ability of an individual to understand speech, particularly in background noise. Despite the emphasis on perceived performance in noise, speech recognition in routine audiologic practice is measured by word recognition in quiet (WRQ).

Article Synopsis
  • Antiretroviral therapy for children under 3 years with HIV often uses a liquid formulation of lopinavir/ritonavir (LPV/r), but it has taste issues and requires refrigeration, which led to the development of LPV/r oral pellets that can be mixed with food and don't need cooling.
  • The study assessed the drug exposure of these LPV/r oral pellets in Kenyan and Ugandan children using pharmacokinetic modeling to understand how body weight affects drug clearance and effectiveness.
  • Analysis of data from 514 children indicated that the pellets achieved the desired drug levels across different weight bands as defined by the World Health Organization, suggesting they are a suitable treatment option for young children with HIV.

Despite the prevalence of exposure to potentially traumatic events (PTEs) among children involved with the child welfare system (CWS), trauma screening is not yet a common practice. The purpose of this study was to assess the impact of embedding a formal trauma screening process in statewide multidisciplinary evaluations for CWS-involved youth. A retrospective record review was conducted with two random samples of cases reflecting both pre- and postimplementation of formal screening procedures (n = 70 preimplementation, n = 100 postimplementation).

Summary: We report the case of a male patient with papillary thyroid cancer, familial thoracic aortic aneurysm and dissection, and a variation in the MYH11 gene. Because of considerable tumor bulk in the neck that was not resectable, the patient underwent partial resection at age 14 years. Since then, the patient has received only suppressive thyroid hormone therapy.

Article Synopsis
  • Acetaminophen poisoning is primarily treated using N-acetylcysteine (NAC), with a traditional three-bag protocol, but a simpler two-bag protocol has been gaining popularity as an alternative.
  • A review of 12 articles showed that the two-bag protocol is as effective as the three-bag regimen in preventing liver injury, with no significant differences in liver damage rates.
  • The two-bag regimen was associated with fewer non-allergic anaphylactoid reactions compared to the three-bag regimen, indicating it may be a safer option for patients.

Objectives: Brain-limited pathogenic somatic variants are associated with focal pediatric epilepsy, but reliance on resected brain tissue samples has limited our ability to correlate epileptiform activity with abnormal molecular pathology. We aimed to identify the pathogenic variant and map variant allele fractions (VAFs) across an abnormal region of epileptogenic brain in a patient who underwent stereoelectroencephalography (sEEG) and subsequent motor-sparing left frontal disconnection.

Methods: We extracted genomic DNA from peripheral blood, brain tissue resected from peri-sEEG electrode regions, and microbulk brain tissue adherent to sEEG electrodes.

Background: A safe and efficacious hemostatic product with a long shelf-life is needed to reduce mortality from hemorrhage due to trauma and improve surgical outcomes for persons with platelet deficiency or dysfunction. Thrombosomes, a trehalose-stabilized, leukoreduced, pooled blood group-O freeze-dried platelet-derived hemostatic (FPH) with a 3-year shelf-life, may satisfy this need.

Objectives: To characterize the mechanism of action of FPH.

Article Synopsis
  • Leiomyosarcoma with adipocytic differentiation, or lipoleiomyosarcoma, is a rare type of tumor found in the female reproductive system, and this study analyzed its characteristics in a series of cases to better understand this condition.
  • Six tumors from six patients were studied, showing different kinds of lipoleiomyosarcoma, with patient ages between 41 and 64, and primary tumors located mainly in the uterine corpus.
  • The study found similarities in immunohistochemical markers and genetic alterations between lipoleiomyosarcomas and non-adipocytic gynecologic leiomyosarcomas, indicating they may share underlying characteristics despite their different appearances.

Leiomyomas with adipocytic differentiation typically occur in the uterus although they may arise at several sites in the female genital tract. While these are most commonly spindled leiomyomas with a component of adipocytic tissue ("conventional lipoleiomyomas"), there is a relatively ill-defined assortment of leiomyoma variants with adipocytic differentiation. We performed a morphologic, immunohistochemical and MDM2 gene amplification analysis of a large series of gynecologic leiomyomas with adipocytic differentiation to better define the clinicopathologic spectrum.
