Case management quality-of-life outcomes for adults with a disability.

The increasing number of working-age adults with a long-term disability has created a need for case management focused on quality-of-life issues as well as functionality. An adaptation of the Quality Health Outcomes Model was used to explore relationships between case manager types and services and characteristics of adults with a disability. Outcome measures included social participation, health maintenance behaviors, and economic productivity.
