Unlabelled: A 66-year-old man presented with abdominal distension and pedal oedema for the past 10 years. He had history of right heart failure on several occasions in the past, and one of these admissions prompted referral to a cardiac specialist. On examination, he had markedly elevated jugular venous pressure with prominent 'y-descent', a tricuspid regurgitation murmur, gross ascites and pedal oedema.

Kawasaki disease is a febrile vasculitis affecting young children, which may lead to coronary aneurysms. Echocardiography, although sensitive in detecting coronary aneurysms, has a limited role in diagnosing coronary thrombus. Here we report the case of a 10-year-old boy who presented with typical features of Kawasaki disease with giant coronary aneurysms.

A 48-year-old man presented with chronic right sided heart failure. 2D echocardiography revealed the classical features of left ventricular endomyocardial fibrosis with a prominent right ventricular apical aneurysm. Right ventriculography further defined the aneurysm.
