Left ventricular (LV) longitudinal myocardial dysfunction can be observed even in type 2 diabetes mellitus (DM) (T2DM) patients with preserved LV ejection fraction (LVEF), and is considered the earliest marker of DM-related cardiac dysfunction. Furthermore, diabetic nephropathy (DN), a common complication in DM, is strongly associated with LV longitudinal myocardial function in T2DM patients, but its association with type 1 DM (T1DM) has not been fully investigated. We studied 125 asymptomatic T1DM patients with preserved LVEF, and 75 age-, gender-, LVEF-matched non-diabetic healthy controls.

Endoscopic resection for GIST has become more widespread in recent years because it is less invasive than surgery. However, when endoscopic resection is performed, a full-layer resection of the gastric wall is often necessary, and extensive suturing is required if perforation occurs, which is a technically challenging procedure. Recently, we reported a new method called endoscopic inversion and strangulation of the muscle layer and resection (EISMR), which consists of endoscopically inverting the muscle layer into the gastric lumen and strangulating the muscle layer with a detachable snare, followed by resection.

Background: Global longitudinal strain (GLS) is reportedly a sensitive marker for early subtle abnormalities in left ventricular (LV) performance of asymptomatic patients with severe aortic stenosis (AS) and preserved LV ejection fraction (LVEF). For symptomatic patients with severe AS and preserved LVEF, however, the association of immediate improvement in GLS after transcatheter aortic valve implantation (TAVI) with long-term outcomes remains uncertain.

Methods: This study concerned 151 symptomatic patients with severe AS and preserved LVEF who had undergone TAVI.

The human gastrointestinal tract, which constitutes the digestive system, contains a large number of virus particles that maintain organizational homeostasis and health. Conversely, viral pathogens have also attracted attention for their involvement in the pathogenesis of certain cancers, including gastrointestinal cancers. To aid prevention and treatment of these cancers, the relevance of gastrointestinal viral factors as potential risk factors needs to be carefully investigated.

Article Synopsis
  • The study examines the impact of hypertension on the risk of left ventricular (LV) dysfunction in patients with malignant lymphoma undergoing anthracycline chemotherapy, particularly doxorubicin.
  • It found that hypertensive patients experienced LV dysfunction at a lower cumulative dose of doxorubicin (259.3 mg/m) compared to non-hypertensive patients (358.9 mg/m).
  • Additionally, for hypertensive patients with LV hypertrophy, the threshold for LV dysfunction was even lower, at just 40.1 mg/m, highlighting the need for careful management in these cases.

  • A nationwide survey in Japan aimed to identify the clinicoepidemiological traits of immunoglobulin G4 (IgG4)-related disease (IgG4-RD) in patients with cancer.
  • The study assessed various types of IgG4-RD, finding autoimmune pancreatitis most prevalent, while highlighting that IgG4-RD patients had a significantly higher cancer prevalence compared to the general population.
  • Notably, the research revealed that 30.5% of cancer cases were diagnosed before the IgG4-RD diagnosis, suggesting a potential connection between the two conditions.

Background: Left ventricular (LV) involvement in diabetic cardiomyopathy has been reported; however, only limited data exist on right ventricular (RV) involvement. Therefore, our purpose was to investigate RV systolic dysfunction and its association with LV longitudinal myocardial dysfunction in patients with type 2 diabetes mellitus (T2DM) and preserved LV ejection fraction (LVEF).

Methods: We studied 177 T2DM patients with preserved LVEF and 79 age-, sex-, and LVEF-matched healthy volunteers.

  • A computer-aided diagnosis (CADx) system was developed to help identify colorectal lesions using two classification methods: the NICE and JNET systems.
  • The CADx systems were trained on NBI (narrow band imaging) images, achieving high accuracy rates of around 97% for the NICE classification and varied but generally strong performance for the JNET classification.
  • The results suggest that these CADx systems can assist non-expert endoscopists in accurately diagnosing colorectal lesions, thereby improving diagnostic consistency and education.

  • - The study examines the effects of closing atrial septal defects (ASD) on right ventricular overload and functional capacity in adults, finding that some patients remain symptomatic despite closure due to delays in treatment.
  • - Researchers analyzed data from 120 ASD patients and 39 controls, focusing on right ventricular global longitudinal strain (RV-GLS) and the TAPSE/PASP ratio to assess heart function before and after closure.
  • - Findings indicate that preoperative TAPSE/PASP ratio and pulmonary vascular resistance index were significant predictors of postoperative symptoms, highlighting their importance in evaluating treatment outcomes for ASD patients.

Article Synopsis
  • The aging process significantly increases the risk of heart failure, especially heart failure with preserved ejection fraction (HFpEF), which affects older individuals more than younger ones.
  • A study involved 102 participants, including 25 with HFpEF and 77 healthy controls, divided into three age groups to assess cardiovascular response to stress through echocardiography.
  • Findings showed that while healthy individuals experienced improved heart function with age-related stress, those with HFpEF did not, indicating that left ventricular remodeling and reduced preload reserve play crucial roles in distinguishing HFpEF from normal aging.

  • This study investigates the haemodynamic response of patients with heart failure with reduced ejection fraction (HFrEF) to non-invasive preload stress through a combination of semi-sitting and passive leg-lifting positions.
  • 101 HFrEF patients and 35 healthy controls were tested, revealing that changes in stroke volume index (SVi) during postural stress could help identify patients at higher risk for future cardiovascular events.
  • The results suggest this method is an effective and safe way to assess heart function and predict potential risks in HFrEF patients, making it a valuable clinical tool.

Article Synopsis
  • Left ventricular (LV) longitudinal myocardial dysfunction is an early marker of heart issues in type 2 diabetes mellitus (T2DM) patients, and high heart rates (HR) may impact this dysfunction.
  • A study involving 192 T2DM patients and 81 healthy volunteers found that T2DM patients with high HR had significantly lower global longitudinal strain (GLS), indicating poorer heart function.
  • High HR was independently linked to subclinical LV dysfunction in T2DM, suggesting that monitoring HR could be crucial for managing heart health in these patients.

The 6th World Symposium on Pulmonary Hypertension proposed that precapillary pulmonary hypertension (PH) be defined as mean pulmonary arterial pressure (mPAP) >20 mmHg instead of mPAP ≥25 mmHg. Peak tricuspid regurgitation velocity (TRV) >3.4 m/s is widely used to predict PH, but it is unclear whether this value remains reliable for the new definition of PH.
