There are cases of idiopathic interstitial pneumonias (IIPs) that do not meet the diagnostic criteria for connective tissue disease but have clinical features suggestive of autoimmune process. Interstitial pneumonia with autoimmune features (IPAF) was recently proposed as a research concept for these patients. Although several prospective studies on IPAF have been conducted, its clinical significance in cryptogenic organizing pneumonia (COP) remains unclear.

Background: Adding bevacizumab to first-generation epidermal growth factor receptor (EGFR)-tyrosine kinase inhibitors (TKIs) prolonged the progression-free survival (PFS), but limited data are available for second-generation EGFR-TKIs. AfaBev-CS is a randomized, phase II trial comparing afatinib plus bevacizumab and afatinib alone as first-line treatment.

Patients And Methods: Untreated patients with non-squamous non-small cell lung cancer (NSCLC) harboring EGFR mutations (Del19 or L858R) were enrolled and randomly assigned to receive either afatinib (30 mg) plus bevacizumab (AfaBev group) or afatinib (40 mg) monotherapy (Afa group).

Article Synopsis
  • Focal segmental glomerulosclerosis (FSGS) leads to kidney problems due to damage in podocytes, with specific genetic variants in the INF2 gene linked to different disease outcomes.
  • The research analyzed how these INF2 variants affect cell structures, finding that those linked to Charcot-Marie-Tooth (CMT) and FSGS combined showed more severe cellular issues than those causing only FSGS.
  • Findings suggest that the CMT/FSGS variants disrupt the organization of the cytoskeleton and organelle interactions more significantly than the variants responsible solely for monogenic FSGS, indicating a need for further research into specific cellular pathways.

We encountered two consecutive cases of adult-onset Kawasaki disease (AKD) that were difficult to diagnose. In both cases, Kawasaki disease was not considered as a differential diagnosis in the early stages. However, it was possible to make a diagnosis by citing the disease as a differential diagnosis and introducing the patients to the pediatrics department.

A 3-year-old neutered male golden retriever administered zonisamide for the treatment of seizures showed lethargy and had normal anion gap metabolic acidosis with hypokalaemia, hyperchloremia, and alkaline urine. The serum zonisamide concentration was close to the upper limit, which raised a suspicion of adverse effects of zonisamide. This is the first report showing that the fractional excretion of bicarbonate after compensation for the plasma bicarbonate concentration by a sodium bicarbonate infusion was approximately 5%, indicating distal renal tubular acidosis (RTA).

Article Synopsis
  • The study aimed to evaluate how different doses of pimobendan affect the heart's pumping ability in healthy cats and assess the relationship between heart function and the levels of pimobendan and its active metabolite, ODMP, in the bloodstream.
  • Five healthy cats underwent four different dosing protocols of pimobendan over 14 days, while cardiac function was monitored through echocardiography and blood tests to measure drug concentration.
  • Results showed that higher doses of pimobendan significantly improved heart function, with notable correlations found between heart performance and plasma levels of both pimobendan and ODMP.

We examined the chemical constitution of the red alga Laurencia saitoi Perestenko, collected from Katsuura, Boso Peninsula, Chiba Prefecture, Japan. This specimen produced a new polyhalogenated acetogenin, named katsuurallene (1), which structure was determined by the spectral methods, along with known diterpene, deoxyparguerol (2) and triterpene, thyrsiferol (3). In this paper we describe the structural elucidation of katsuurallene together with some biological activities.

Lignin, a complex aromatic polymer, represents a significant obstacle in lignocellulosic biomass utilization. The polymerization of lignin occurs by radical couplings, which mainly form ether and C-C bonds between monolignol units. The chemical stability of these bonds between monolignol units causes the recalcitrant nature of lignin.
