A geriatric patient with major burns:case report.

As is predictable, mortality and morbidity among geriatric patients are higher in patients with major burns. Decreased radiopulmonary reserves and malnutrition characterized by protein/energy deficiency and ageing of skin are predisposing factors which increase mortality and morbidity. In this study, we present a 90-yr-old patient with 46% total body surface area of 2nd-3rd degree burns.

Surgical treatment of liver hydatid cysts.

Background: The surgical treatment technique for liver hydatic cyst (LHC) cannot be standardized, and the surgical technique should be tailored according to the extent of the cyst and any adjunct complications of hydatid disease.

Patients And Methods: All patients were treated with albendazole (10 mg/kg/day) for 15 days preoperatively. Total pericystectomy was performed in three patients (7%).

Preventable thermal burns associated with the ignition of paint thinner: experience of a burn care network in Turkey.

Aim: This retrospective study evaluated the epidemiology of burn injuries, due to paint thinner ignition, in patients treated at the burn units of a university hospital network.

Methods: From 1997 to 2005, 28 patients with thermal burns caused by ignition of paint thinner were admitted to our burn units. Age, sex, etiologic factors, extent and localization of burns, length of hospitalization, outcomes compared with other causes of thermal burns, and mortality rates were recorded for each patient.

An open-label long-term naturalistic study of mirtazapine treatment for depression in cancer patients.

Background: Depression is quite common among cancer patients. It has yet to be determined which antidepressant is the most effective in cancer patients with depression.

Methods: The present study included 21 consecutive depressed oncology patients of whom 19 were followed up for 6 months.

Laparoscopic management of gallbladder duplication.

Gallbladder duplication is a rare condition. Because laparoscopic cholecystectomy is the primary treatment modality for the diseased single gallbladders, it should be the choice of treatment for double gallbladders. However, preoperative imaging methods may be unsatisfactory for the correct diagnosis.

Urinary annexin V in children with nephrotic syndrome: a new prognostic marker?

Annexin V has a molecular weight of 32-35 kDa and has been reported to possess anticoagulant activity, inhibition of phospholipase A(2), regulation of membrane transport, proliferation and signal transduction. It is reported that urinary annexin V concentration may be an indicator of apoptosis and acute renal injury related to the urinary protein level. The aim of this study was to define the role of urinary annexin V excretion and serum annexin V concentrations as new prognostic tools and follow-up criteria in children with steroid-sensitive (SSNS) and steroid-resistant nephrotic syndrome (SRNS).

Adhesion molecules as prognostic markers in pancreatic adenocarcinoma.

Background And Objectives: Pancreatic adenocarcinoma is a highly aggressive cancer with high metastatic potential and therefore, a high mortality rate. Ezrin, radixin, moesin, and E-cadherin are transmembrane glycoproteins that regulate cell motility, migration, and metastasis. In this study, we investigated the relationship of ezrin, moesin, and E-cadherin expression with the clinicopathological features of pancreatic ductal adenocarcinoma.

Comparison of the effects of desflurane and sevoflurane on the expression of platelet surface glycoproteins in unstimulated and adenosine diphosphate-induced platelets in vitro.

Study Objective: To compare the effects of one minimum alveolar concentration (MAC) desflurane and sevoflurane on the expression of CD42b (glycoprotein [GP] Ib), CD41 (GPIIb), CD61 (GPIIIa), CD62P (P-selectin), and CD63 in both unstimulated and adenosine diphosphate (ADP)-stimulated platelets in vitro.

Reconstruction of distal forefoot burn defect with retrograde medial plantar flap.

Burn reconstruction of forefoot remains as a difficult challenge, because the local flap alternatives are limited. We evaluated the efficiency of distally based medial plantar fasciocutaneous island flap in the coverage of forefoot defects resulting from release of toe contracture and burn debridement. Four patients with toe contractures and two patients with third degree burn in forefoot were treated between June 2004 and February 2006.

Urgent adult-to-adult right lobe living donor liver transplantation for acute liver failure; a single center experience from Turkey.

Background/aims: Acute liver failure is a fatal condition unless an urgent liver transplantation is performed. In countries like Turkey, because of limited availability of cadaveric allografts, living donors could be used as an organ source for acute liver failure. We report our single center experience.

The adequacy of laparoscopy for continuous ambulatory peritoneal dialysis procedures.

The aim of this study was to determine the safety and efficacy of diagnostic/therapeutic laparoscopy in the management of peritoneal Tenchoff catheter placement in end-stage renal disease patients who had previous abdominal surgery and malfunctioning peritoneal dialysis catheters. From 1999 to 2004, 16 videolaparoscopic procedures were performed in 16 patients who had previous laparotomies. Laparoscopy was performed before peritoneal catheter placement in seven (group 1) and in 9 patients with peritoneal dialysis catheters in place, laparoscopy was performed for the management of catheter dysfunction (group 2).

Treatment of gingival recession using enamel matrix proteins: a case report with 4-year follow-up.

Obtaining predictable and optimal coverage of exposed root surfaces and correction of corresponding gingival recessions have become important goals of periodontal plastic surgery. Various surgical techniques have been proposed for coverage of root surfaces. A therapeutic advantage may be gained if periodontal regeneration is obtained in addition to coverage of root with gingiva.

Reno-vascular hypertension in childhood: a nationwide survey.

Renovascular disease accounts for 8-10% of all cases of paediatric hypertension, whereas, in adults, its incidence is approximately 1%. The Turkish Paediatric Hypertension Group aimed to create the first registry database for childhood renovascular hypertension in Turkey. Twenty of the 28 paediatric nephrology centres in Turkey responded to the survey and reported 45 patients (27 girls, 18 boys) with renovascular hypertension between 1990 and 2005.

Giant hepatic hydatid cyst: a case report.

The effect of metoclopramide on gastric emptying in traumatic brain injury.

Objective: Gastric paresis in traumatic brain injury (TBI) hinders the effectiveness of enteral support in this patient group. In this study we have investigated the effect of metoclopramide on gastric emptying in TBI patients.

Method: In this prospective, randomized, controlled, double-blind study, 19 TBI patients with Glasgow Coma Scale scores of 3-11 were included.

A case of Cauda Equina syndrome in a leukemic patient due to intrathecal methotrexate.

The leukemias may cause neurologic dysfunction through either direct invasion of the nervous system or indirectly through cytopenias, or it may occur as a result of the necessarily vigorous treatment programs for leukemia. We report here a 24-year-old acute lymphoblastic leukemia patient who in her second cycle of hyper-CVAD chem therapy regimen (high-dose Ara-C and high-dose methotrexate) received intrathecal methotrexate and two days afterwards was diagnosed as having Cauda Equina syndrome (CES). A lumbo-sacral MRI imaging with gadolinium was performed and there was a remarkable enhancement in the Cauda Equina region, suggesting either leukemic involvement or a type of neurologic complication associated with intrathecal methotrexate treatment.

Variables affecting patient satisfaction in breast surgery: a cross-sectional sample of Turkish women with breast cancer.

Objective: In Turkey, despite the fact that breast cancer accounts for 24.1% of all cancer in women, a very small number of these patients receive breast reconstruction. This low percentage would seem to indicate that there are several factors affecting the decision of which surgical procedure should be selected.

Ambulatory blood pressure monitoring and renal functions in children with a solitary kidney.

The aim of this study is to investigate the blood pressure (BP) profile, microalbuminuria, renal functions, and relations with remaining normal kidney size in children with unilateral functioning solitary kidney (UFSK). Sixty-six children with UFSK were equally divided into three groups: unilateral renal agenesis (URA), unilateral atrophic kidney (UAK), and unilateral nephrectomy (UNP). Twenty-two age-, weight-, and height-matched healthy children were considered as a control group.

Liver biopsy: is the pain for real or is it only the fear of it?

Although percutaneous liver biopsy (PLB) has very low mortality and morbidity rates, it often is considered painful and frightening by the patients. This study was designed to grade the intensity of pain expected before the procedure and experienced during the procedure, and whether there is any correlation between pain and the emotional state of the patient. A total of 118 consecutive patients (aged 19-68 (mean, 44) years), who were undergoing PLB for the first time, were included in the study.

Complete heart block in a neutropenic patient with invasive aspergillosis.

A patient with leukemia who developed complete heart block after the diagnosis of pulmonary aspergillosis is reported. The patient had probable invasive pulmonary aspergillosis with a sudden tachypnea, dyspnea, fever, bilateral pulmonary infiltrates and acute respiratory insufficiency after chemotherapy. On the sixth day of antifungal therapy, she developed complete atrioventricular block.
