We report a technique for surgical repair of laryngotracheoesophageal clefts from type II to IV through an anterior approach that involves performing a tracheoesophagoplasty with two overlapping flaps (TEP-TOF). We offer a technical description, a retrospective study, and a review of the literature. Ten children were operated on for laryngotracheoesophageal clefts of types II, III, or IV of the Benjamin and Inglis classification by the TEP-TOF procedure.

Objective: To report two cases of an undescribed association of first branchial cleft fistula and middle ear congenital cholesteatoma and to discuss management and embryological hypothesis.

Results: Both patients were young girls free of past medical or surgical history. Surgical removal of the first cleft anomaly found in the two cases a fistula routing underneath the facial nerve.

This prospective study describes the clinical utility of virtual endoscopy based on geometric surface modeling of the laryngotracheal lumen. Eighteen children with dyspnea related to either subglottic angioma (n = 5) or laryngotracheal stenosis (n = 13) were included. All patients underwent video-recorded operative endoscopy, spiral computed tomography, and 3-dimensional reconstruction of the laryngotracheal lumen.

Tracheomalacia is characterized by an expiratory tracheal collapse. The revealing clinical features usually occur after a symptom-free period during the first year of life (during the first three months in 60% of cases): mainly stridor, wheezing, chronic cough, apnoeic attacks, and difficult breathing. Tracheoscopy allows the differentiation of primary tracheomalacia and tracheomalacia secondary to pressure on the trachea from the vascular ring or mediastinal tumour.

Patients And Methods: The files of 12 children presenting with a sternocleidomastoid tumor of infancy at the Timone Children's Hospital in Marseille between 1990 and 1999 were retrospectively studied. All of them underwent physical and ultrasonographic examination.

Results: The mass was firm, within the lower two-thirds of the sternocleidomastoid muscle.

Endoscopy is an essential stage in the management of laryngo-tracheal disorders in the infant, but remains a difficult procedure. CT scanning is used to confirm the abnormality and to estimate the degree to which the airway is compromised. The authors describe virtual endoscopy of the larynx and trachea based on a mathematical model, itself constructed from CT images.
