Digital redesign of the radiotherapy course for medical students with a blended learning approach.

Purpose: Due to the need for high-quality teaching, we present a new blended learning concept combining digital modules, interactive seminars, and clinical experience. Furthermore, we evaluated its acceptance among students.

Methods: A new concept for teaching was applied to the radiotherapy module and made available via the Hannover Medical School (MHH) learning management system as part of a blended learning concept with educational films, multimedia learning modules, online seminars, and onsite practical training.

[Educational film on the geriatric basic assessment in primary care-Methods of cinematic authenticity].

Background: The relevance of care of older persons in general practice requires the conveyance of procedural and conditional knowledge of the geriatric basic assessment (GBA) even during medical studies. There is a need for action with respect to student knowledge on specific problems of older patients. This paper describes how the primary care situation can be made tangible for students based on a film project at the Hannover Medical School (MHH).

The strength of the meridional overturning circulation of the stratosphere.

The distribution of gases such as ozone and water vapour in the stratosphere - which affect surface climate - is influenced by the meridional overturning of mass in the stratosphere, the Brewer-Dobson circulation. However, observation-based estimates of its global strength are difficult to obtain. Here we present two calculations of the mean strength of the meridional overturning of the stratosphere.

Various Cinematic Forms of Educational Videos About the Physical Examination - Are There Differences in the Evaluation by Medical Students?

To improve medical students' competences in physical examination videos clips were created, with and without an explaining commentary. The uncommented videos show the communication and interaction between physician and patient during a physical examination, the commented videos show the single steps of the physical examination supplemented with an off-screen commentary emphasizing important facts. To investigate whether uncommented and more authentic videos are more helpful to practice a physical examination than commented videos we interviewed 133 students via online surveys.

UTLS water vapour from SCIAMACHY limb measurementsV3.01 (2002-2012).

The SCanning Imaging Absorption spectroMeter for Atmospheric CHartographY (SCIAMACHY) aboard the Envisat satellite provided measurements from August 2002 until April 2012. SCIAMACHY measured the scattered or direct sunlight using different observation geometries. The limb viewing geometry allows the retrieval of water vapour at about 10-25 km height from the near-infrared spectral range (1353-1410 nm).

Wilderness Emergency Medical Services Medical Director Course: core content developed with Delphi technique.

The National Association of Emergency Medical Services Physicians' (NAEMSP) position on the role of medical oversight within an operational Emergency Medical Service (EMS) program highlights the importance of integrating specially trained medical directors within the structure of these programs. In response, the NAEMSP Wilderness EMS (WEMS) Committee recognized the need for the development of an educational curriculum to provide physicians with the unique skills needed to be a medical director for a WEMS agency. This paper describes the Delphi process used to create the subject matter core content, as well as the actual core content developed.

In an effort to produce best practice guidelines for spine immobilization in the austere environment, the Wilderness Medical Society convened an expert panel charged with the development of evidence-based guidelines for management of the injured or potentially injured spine in an austere (dangerous or compromised) environment. Recommendations are made regarding several parameters related to spinal immobilization. These recommendations are graded on the basis of the quality of supporting evidence and balance between the benefits and risks or burdens for each parameter according to the methodology stipulated by the American College of Chest Physicians.

Optimized spectral microwindows for data analysis of the Michelson Interferometer for Passive Atmospheric Sounding on the Environmental Satellite.

For data analysis of the Michelson Interferometer for Passive Atmospheric Sounding (MIPAS) atmospheric limb emission spectroscopic experiment on Environmental Satellite microwindows, i.e., small spectral regions for data analysis, have been defined and optimized.

Optimized forward model and retrieval scheme for MIPAS near-real-time data processing.

An optimized code to perform the near-real-time retrieval of profiles of pressure, temperature, and volume mixing ratio (VMR) of five key species (O(3), H(2)O, HNO(3), CH(4), and N(2)O) from infrared limb spectra recorded by the Michelson Interferometer for Passive Atmospheric Sounding (MIPAS) experiment on board the European Space Agency (ESA) Environmental Satellite ENVISAT-1 was developed as part of a ESA-supported study. The implementation uses the global fit approach on selected narrow spectral intervals (microwindows) to retrieve each profile in sequence. The trade-off between run time and accuracy of the retrieval was optimized from both the physical and the mathematical points of view, with optimizations in the program structure, in the radiative transfer model, and in the computation of the retrieval Jacobian.

Sensitivity in detecting free intraperitoneal fluid with the pelvic views of the FAST exam.

The multiple-view focused assessment with sonography for trauma (FAST) exam is an integral tool in the assessment of blunt abdominal trauma. A prospective observational study was performed to compute the average minimum volume of detectable intraperitoneal fluid with the pelvic views of the FAST exam. All adult patients from October 1999 to May 2001, who presented to the ED with blunt abdominal trauma and underwent a clinically indicated diagnostic peritoneal lavage (DPL), were candidates for admission to the pelvic ultrasound study.

Neoadjuvant chemotherapy, radical resection with intraoperative radiation therapy (IORT): improved treatment for gastric adenocarcinoma.

Background: Adenocarcinoma of the stomach and gastroesophageal junction results in substantial morbidity, locoregional recurrence, and death. Surgical procedures, even with adjuvant therapy, have not significantly improved survival. This study evaluated the toxicity, response rate, locoregional control, and survival of patients with locally advanced gastric cancer that was treated with neoadjuvant multimodality therapy.

An alternative approach: antegrade catheter-directed thrombolysis in a case of phlegmasia cerulea dolens.

Phlegmasia cerulea dolens is an uncommon sequela of severe deep venous thrombosis of the lower extremities. Characterized by massive edema, arterial and venous compromise, and threats to limb and life, this clinical entity is a clear indication for thrombolytic therapy. We report an innovative approach to conventional thrombolysis via a lesser saphenous vein cut-down.

Regulation of phosphoenolpyruvate carboxykinase by glutamine and ATP as possible control mechanisms of renal gluconeogenesis.

In different metabolic states renal phosphoenolpyruvate carboxykinase (PEP-CK) activities are closely correlated with in vitro glucogenic rates, suggesting a limitation of the glucogenic capacity of kidney by this enzyme. Stimulation of renal gluconeogenesis from pyruvate, lactate, and succinate by lysine and glutamine was therefore associated with a regulatory attack of these amino acids at the level of PEP-carboxykinase. This postulate was confirmed by the failure of lysine to stimulate glucose synthesis from fructose.
