The size of local recurrence influences outcomes in chondrosarcoma: Significant impact observed in grade 2 tumours.

Background: Chondrosarcoma is the second most common bone malignancy. Chondrosarcoma is considered a surgical disease, and the role of surgical margin is important since local recurrence is known to decrease survival. The aim of this study was to investigate the timing of local recurrence and the size of local recurrence in post local recurrence survival in chondrosarcoma.

A Rare Case of Scrotal Swelling: Prostate Cancer Metastasis Masquerading as a Complicated Hydrocele.

Metastasis of prostate cancer to the testes is exceptionally rare. We report the case of a 67-year-old male with a 10-year history of high-risk prostate cancer, previously treated and currently in remission, who presented with left scrotal swelling. The swelling was clinically and radiologically diagnosed as a hydrocele and treated surgically.

Implementation of Integrated Clinical Pharmacogenomics Testing at an Academic Medical Center.

Background: Pharmacogenomics has demonstrated benefits for clinical care, including a reduction in adverse events and cost savings. However, barriers in expanded implementation of pharmacogenomics testing include prolonged turnaround times and integration of results into the electronic health record with clinical decision support. A clinical workflow was developed and implemented to facilitate in-house result generation and incorporation into the electronic health record at a large academic medical center.

Hippocampal atrophy over two years in relation to tau, amyloid-β and memory in older adults.

In this longitudinal brain imaging study, we aimed to characterize hippocampal tau accumulation and subfield atrophy relative to cortical amyloid-β and memory performance. We measured tau-PET in regions associated with Braak stages I to VI, global amyloid-PET burden, hippocampal subfield volumes and memory assessments from 173 participants aged 55-85. Eighty-six of these participants were tested again two years later.

Enhancing surgical margins: A key to survival in pelvic chondrosarcoma.

Introduction: Pelvic chondrosarcoma, originating in the cartilaginous tissue of the pelvis, presents unique surgical challenges due to its proximity to critical neurovascular structures and essential organs. The aim of this study was to explore the effect of changes in practice and improvements in understanding on the outcomes for patients undergoing surgery for pelvic chondrosarcoma.

Material And Methods: The study comprised a retrospective analysis of 216 patients making up 2 patient cohorts drawn from a single centre over 2 time periods (2003-2012 and 2013-2022).

Taxonomy-based prompt engineering to generate synthetic drug-related patient portal messages.

Objective: The objectives of this study were to: (1) create a corpus of synthetic drug-related patient portal messages to address the current lack of publicly available datasets for model development, (2) assess differences in language used and linguistics among the synthetic patient portal messages, and (3) assess the accuracy of patient-reported drug side effects for different racial groups.

Methods: We leveraged a taxonomy for patient- and clinician-generated content to guide prompt engineering for synthetic drug-related patient portal messages. We generated two groups of messages: the first group (200 messages) used a subset of the taxonomy relevant to a broad range of drug-related messages and the second group (250 messages) used a subset of the taxonomy relevant to a narrow range of messages focused on side effects.

Protocol: What works to increase the use of evidence for policy decision-making: A systematic review.

This is the protocol for a Campbell systematic review. The objectives are as follows: Our aim is to collect, assess, and synthesise all the available empirical evidence on what works to support evidence-informed decision-making by policymakers. In doing so, we will aim to answer the following research questions: What are the impacts of interventions to support evidence-informed decision-making by policymakers? What are the factors which have influenced the impact of these interventions, and their design and implementation in low- and middle-income countries? In answering these questions, our goal is to estimate the overall impact and relative effectiveness of different interventions, identify factors or configurations of factors that support or hinder their effectiveness in low- and middle-income countries and to identify gaps and areas for future primary research.

The Pharmacogenomics Global Research Network Implementation Working Group: global collaboration to advance pharmacogenetic implementation.

Pharmacogenetics promises to optimize treatment-related outcomes by informing optimal drug selection and dosing based on an individual's genotype in conjunction with other important clinical factors. Despite significant evidence of genetic associations with drug response, pharmacogenetic testing has not been widely implemented into clinical practice. Among the barriers to broad implementation are limited guidance for how to successfully integrate testing into clinical workflows and limited data on outcomes with pharmacogenetic implementation in clinical practice.

Low-dose and ultra-low-dose estradiol and dydrogesterone in postmenopause: an analysis by body mass index.

Objective: Oral, low-dose and ultra-low-dose continuous combined 17β-estradiol (E) plus dydrogesterone (D) reduce vasomotor symptoms (VMS) in postmenopausal women.

Methods: Two phase 3, double-blind studies were included. In the European study, postmenopausal women were randomized 2:1:2 to receive E0.

Plasma phosphorylated tau181 outperforms [F] fluorodeoxyglucose positron emission tomography in the identification of early Alzheimer disease.

Background And Purpose: This study was undertaken to compare the performance of plasma p-tau181 with that of [F]fluorodeoxyglucose (FDG) positron emission tomography (PET) in the identification of early biological Alzheimer disease (AD).

Methods: We included 533 cognitively impaired participants from the Alzheimer's Disease Neuroimaging Initiative. Participants underwent PET scans, biofluid collection, and cognitive tests.

The non-inferiority of piperonyl-butoxide Yorkool G3 insecticide-treated nets compared to Olyset®Plus measured by Anopheles arabiensis mortality in experimental huts in Tanzania.

Background: Non-inferiority trials are recommended by the World Health Organization (WHO) to demonstrate that health products show comparable efficacy to that of existing standard of care. As part of the WHO Global Malaria Programme (GMP) process of assessment of malaria vector control products, a second-in-class insecticide-treated net (ITN) must be shown to be non-inferior to a first-in-class product based on mosquito mortality. The public health impact of the first-in-class pyrethroid-piperonyl butoxide (PBO) ITN, Olyset Plus, has been demonstrated in epidemiological trials in areas with insecticide-resistant mosquitoes, but there is a need to determine the efficacy of other pyrethroid-PBO nets to ensure timely market availability of nets in order to increase access to ITNs.

Are Positive Biopsy Margins in Melanoma Significant?: A Cohort Study of Micro- Versus Macroscopic Margin Status and Their Impact on Residual Disease and Survival.

Background: Presence of positive biopsy margins in melanoma can provoke anxiety over potential disease progression from delays to surgical excision, but their impact on outcomes is unknown. We aimed to compare the presence of residual melanoma in the surgical excision specimen and survival between patients with negative, microscopically positive, and macroscopically positive biopsy margins.

Methods: Patients with cutaneous melanoma who underwent surgical excision over a 13-year period were included.

Folates are B-group vitamins that function in one-carbon metabolism. Here we show that a specific folate can activate serotonergic neurons in C. elegans to modulate behavior through a pathway that requires the folate receptor FOLR-1 and the GON-2 calcium channel.

Staged revision of the infected knee arthroplasty and endoprosthesis.

Aims: Periprosthetic joint infection (PJI) is a challenging complication of any arthroplasty procedure. We reviewed our use of static antibiotic-loaded cement spacers (ABLCSs) for staged management of PJI where segmental bone loss, ligamentous instability, or soft-tissue defects necessitate a static construct. We reviewed factors contributing to their failure and techniques to avoid these complications when using ABLCSs in this context.

A pictographic guide for decision making in surgery for pelvic bone sarcoma.

Pelvic bone sarcoma surgery is challenging due to complex anatomy, proximity to major neurovascular structures, and, more importantly, the potential for complications. Decision-making is vital in offering patients the best oncological and functional outcomes after surgery. Multidisciplinary teams involved from the stage of diagnosis and treatment planning, followed by surgery by experienced teams have proven to be beneficial.

Evaluation of Anopheline Diversity and Abundance across Outdoor Collection Schemes Utilizing CDC Light Traps in Nchelenge District, Zambia.

In the global fight against malaria, standard vector control methods such as indoor residual spraying (IRS) and insecticide-treated nets (ITNs) are intended to protect inside residential structures and sleeping spaces. However, these methods can still leave individuals vulnerable to residual transmission from vectors that they may be exposed to outdoors. Nchelenge District in northern Zambia experiences persistently high malaria transmission even with ITNs and IRS in place.

Modeling the progression of neuropsychiatric symptoms in Alzheimer's disease with PET-based Braak staging.

In Alzheimer's disease (AD), neuropsychiatric symptoms (NPS) correlate with tau deposition in the brain. Here, we investigated the association of PET-based Braak stages with NPS and assessed whether they predict annual changes in NPS. We evaluated 231 individuals in the aging and AD continuum.
