The article is devoted to pharmacoeconomic aspects of treatment of arterial hypertension including its combination with diabetes mellitus. Data of four studies of comparison of clinical and economical effectiveness of treatment with original enalapril and its fixed combination with hydrochlorothiazide with some generic preparations of enalapril are presented. It has been shown that original enalapril has advantages over generics and from position of treatment cost.

Background: Aim of this study was to evaluate adjuvant magnesium orotate on mortality and clinical symptoms in patients with severe heart failure under optimal cardiovascular medication.

Methods: In a monocentric, controlled, double-blind study, 79 patients with severe congestive heart failure (NYHA IV) under optimal medical cardiovascular treatment were randomised to receive either magnesium orotate (6000 mg for 1 month, 3000 mg for about 11 months, n = 40) or placebo (n = 39). Both groups were comparable in demographic data, duration of heart failure and pre- and concomitant treatment.

The vegetative regulation of heart rhythm was estimated from mathematical analysis of heart rhythm in 79 patients with prolapse of mitral valve (PMV) and in 15 patients with abnormal position of chordae (ADC). The sympathetic tone elevation was more typical for patients with HMV. Meanwhile the decrease of sympathetic tone was observed in patients with PMV and ADC with vegetative crises in anamnesis.

Phenotype was investigated in 86 patients with idiopathic prolapses of atrioventricular valves and in 21 patients with displaced chordae. 10 and 4 markers of connective tissue dysplasia were distinguished for patients with the prolapses and anomalous chordae, respectively. For patients with dysplasia of connective tissue of the heart a modified version of phenotypical length is proposed: norm-anomalous chordae--prolapse of the 1st degree--prolapse of the 2nd degree.

A follow-up study in the blood of the total, free and bound 11-oxycorticosteroids (11-OCS) and reserve binding capability of transcortin (RBCT) in 246 patients with acute myocardial infarction, complicated in 127 with cardiogenic shock, in 62 by ventricular fibrillation and in 57 by cardiac rupture was undertaken. In patients with acute myocardial infarction there was statistically reliable increased activation of glucocorticoid function of the adrenals with simultaneous marked drop of RBCT as compared with patients who had uncomplicated myocardial infarction. The greatest changes were found in patients with true cardiogenic shock and shock with cardiac rupture.

The article deals with the results of study of bound and free 11-oxycorticosteroids and the reserve binding capacity of transcortin in 65 patients with myocardial infarction complicated by various types of cardiogenic shock. The most significant increase in the level of 11-oxycorticosteroids and decrease in transcortin reserve binding capacity were found in patients with true cardiogenic shock.

The results of the study of adrenal cortical function in 94 patients (including 20 patients with acute myocardial infarction and pulmonary edema) and in 20 practically healthy persons are discussed. Marked activation of adrenal cortical hormones was noted in patients with acute myocardial infarction and edema of the lungs.
