Misdiagnosis of myocardial injury in the presence of elevated cardiac troponins and ECG anomalies led to inappropriate management and highlights the difficulty in treating the collapsed athlete following arduous exercise.

Barriers to hospice use among African Americans: a systematic review.

The present review was undertaken to explore recent evidence in the professional literature pertaining to use of hospice services by African Americans. The article addresses the research methods that have been used to study African American hospice use, obstacles to African American participation in hospice that have been identified, and interventions designed to increase the number of African Americans using hospice services that have been tested. Results indicate that both qualitative and quantitative methods have identified the following key factors that contribute to the underuse of hospice services by members of the African American community: personal or cultural values in conflict with hospice philosophy, lack of awareness of hospice services, concern about burdening family, economic factors, mistrust of the health care system, and expected lack of ethnic minority employees in hospice agencies.

Purpose Of Review: To review current knowledge of the relationship between cytokines, the acute phase response (APR) and the development of cachexia.

Recent Findings: Cytokines important in the initiation of the APR are also important in the genesis of cachexia. The presence of an APR or high circulating levels of pro-inflammatory cytokines are known to be related to adverse outcome in cancer patients.

Differentiation of RVOT-VT and ARVC in an elite athlete.

Background: Differentiation of right ventricular outflow tract-ventricular tachycardia (RVOT-VT) and arrhythmogenic right ventricular cardiomyopathy (ARVC) can be problematic in athletes. The high incidence of sudden death as the first clinical manifestation in ARVC highlights the importance of correct diagnosis and treatment. We report on the case of RVOT-VT in an elite female sprinter, and we review the literature on ventricular tachycardia (VT) in the absence of structural heart disease and ARVC.

Glutamate receptor subtypes evidenced by differences in desensitization and dependence on the GLR3.3 and GLR3.4 genes.

Ionotropic glutamate (Glu) receptors in the central nervous system of animals are tetrameric ion channels that conduct cations across neuronal membranes upon binding Glu or another agonist. Plants possess homologous molecules encoded by GLR genes. Previous studies of Arabidopsis thaliana root cells showed that the amino acids alanine (Ala), asparagine (Asn), cysteine (Cys), Glu, glycine (Gly), and serine trigger transient Ca(2+) influx and membrane depolarization by a mechanism that depends on the GLR3.

Choice as an act of meaning: the case of social class.

Social class is one important source of models of agency--normative guidelines for how to be a "good" person. Using choice as a prototypically agentic action, 5 studies test the hypotheses that models of agency prevalent in working-class (WK) contexts reflect a normative preference for similarity to others, whereas models prevalent in middle-class (MD) contexts reflect a preference for difference from others. Focusing on participants' choices, Studies 1 and 2 showed that participants from WK relative to MD contexts more often chose pens that appeared similar to, rather than different from, other pens in the choice set, and more often chose the same images as another participant.

Sensitized airway smooth muscle plasticity and hyperreactivity: a review.

To help elucidate the mechanisms underlying asthmatic bronchospasm, the goal of our research has been to determine whether airway smooth muscle (ASM) hyperreactivity was the responsible factor. We reported that in a canine model of asthma, the shortening capacity (DeltaLmax) and velocity (Vo) of in vitro sensitized muscle were significantly increased. This increase was of sufficient magnitude to account for 75% narrowing of the in vivo airway, but maximal isometric force was unchanged.

Relaxation of airway smooth muscle is an inadequately understood yet critical process that, if impaired, may have significant implications for asthma. Here we explore why relaxation is an important process to consider, how it may determine airway hyperresponsiveness, and some of the factors that influence relaxation of the airway smooth muscle. These include mechanical and biochemical factors such as deep inspirations or large amplitude oscillation of the muscle, plastic properties of the muscle, the load the muscle experiences, calcium, phosphorylation of the myosin light chain, cytoskeletal proteins, and sensitization.

Three experiments assessed the influence of the Ebbinghaus illusion on size judgments that preceded verbal, grasp, or touch responses. Prior studies have found reduced effects of the illusion for the grip-scaling component of grasping, and these findings are commonly interpreted as evidence that different visual systems are employed for perceptual judgment and visually guided action. In the current experiments, the magnitude of the illusion was reduced by comparable amounts for grasping and for judgments that preceded grasping (Experiment 1).

Transforming growth factor-beta as a differentiating factor for cultured smooth muscle cells.

The aim of the present study was to determine whether the development of supercontractile smooth muscle cells, contributing to the nonspecific hyperreactivity of airways in asthmatic patients, is due to transforming growth factor (TGF)-beta. In cultured smooth muscle cells starved by removal of 10% foetal bovine serum for 7 days, growth arrest was seen; 30% became elongated and demonstrated super contractility. Study of conditioned medium suggested that the differentiating factor was TGF-beta.

Hydrothermal chemistry of oligomeric vanadium oxyfluorides.

Nine new vanadium oxyfluorides, containing ten different oligomeric vanadium (oxy)fluoride anions have been prepared by solvothermal synthesis in water-ethylene glycol and using organic cations as co-crystallising agents. Crystal structures are reported for each. Amongst the ten structure types, five represent previously unobserved moieties, including one monomeric unit (cis-[VOF4(H2O)]2-), two dimers ([V2O2F6(H2O)2]2- and [V2O2F8]4-) and two tetramers (both isomers of composition [V4O4F14]6-).

Large outbreaks of Salmonella Typhimurium phage type 135 infections associated with the consumption of products containing raw egg in Tasmania.

This report describes one of the largest egg-associated outbreaks of foodborne illness in Australia for many years. Between June and December 2005, five outbreaks of Salmonella Typhimurium phage type 135 were identified in Tasmania, leading to 125 laboratory-confirmed cases. Public health investigations included case and food handler interviews, cohort studies, environmental health investigations of food businesses, microbiological testing, traceback, and inspections and drag swabbing of an egg farm.

Airway smooth muscle dynamics: a common pathway of airway obstruction in asthma.

Excessive airway obstruction is the cause of symptoms and abnormal lung function in asthma. As airway smooth muscle (ASM) is the effecter controlling airway calibre, it is suspected that dysfunction of ASM contributes to the pathophysiology of asthma. However, the precise role of ASM in the series of events leading to asthmatic symptoms is not clear.

An exploratory study of the hydrothermal chemistry of vanadium in HF solutions has resulted in the preparation of four new vanadium (III) fluorides with chainlike structural motifs. [NH4]2[VF5] (1) and [C2N2H10][VF5] (2) feature infinite chains of trans corner-sharing VF4F2/2 octahedra, [C4N2H6][VF5].H2O (3) has cis corner-sharing [VF4F2/2]infinity chains, and [C10N2H8][VF3] (4) has trans corner-sharing [VF2F2/2]infinity chains bridged into sheets by the 4,4'-bipy linker.

Anomalies of the flexor digitorum superficialis muscle are extremely uncommon and usually present as a painful mass or pseudotumour within the palm. Diagnosis may be difficult because many other soft tissue tumours (lipomas, ganglions, giant cell tumours and hamartomas) may present similarly. Magnetic resonance imaging helps to define the extent and characteristics of this anomalous muscle belly and to distinguish it from a soft tissue sarcoma, whereas plain radiographs are of little value.

A multi-jurisdiction case control study was conducted after an increase of Salmonella Typhimurium phage type 135 notifications (a local designated subgroup) was observed throughout Australia. Calcium entry mediated by GLR3.3, an Arabidopsis glutamate receptor with a broad agonist profile.

The amino acids glutamate (Glu) and glycine (Gly) trigger large, rapid rises in cytosolic Ca(2+) concentration and a concomitant rise in membrane potential (depolarization) in plants. The possibility that plant homologs of neuronal ionotropic glutamate receptors mediate these neuron-like ionic responses was tested in Arabidopsis (Arabidopsis thaliana) seedlings using a combination of Ca(2+) measurements, electrophysiology, and reverse genetics. The membrane depolarization triggered by Glu was greatly reduced or completely blocked in some conditions by mutations in GLR3.
