This investigation challenged the proposition that physician patient-centeredness influences patients' experience-of-care (PEC). A theory-driven, three-factor, multigroup structural equation modeling design, using asymptotic-distribution-free and bootstrap estimation, with two national random and 5,000 bootstrap samples challenged the proposition's plausibility, measurement invariance, replicability, robustness against a competing model, and coherence with theory. The model fit [χ(39) = 28, =.

Background: Along with clinical technical competence, nurses' interpersonal ability influences patient outcomes. Patient-centeredness, "[p]roviding care that is respectful of, and responsive to, individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions" (IOM, 2001, p. 3), is especially important in assuring that African-American Medicare patients achieve the desired outcomes.

Objective. This study examined to what degree patient-centeredness-measured as an underlying ability of obstetrical nurses-influenced Medicaid patients' satisfaction with care in hospital obstetrical units. Design.

This study examined the influence of physicians' and nurses' patient-centeredness on the satisfaction of African American female Medicaid patients. A multigroup structural equation modeling design was used to test the hypothesized model and its stability across national random test (Nt=98) and cross-validation (Ncv=296) samples. The model fit well.

Despite its recognition as a key dimension of healthcare quality, it is often unclear what exactly patient-centeredness means. A generally accepted measurement model of patient-centeredness is still nonexistent, current operational definitions lack sufficient specificity to inform providers how it relates to outcomes, and the influence of patient-centeredness on pediatric patients and families has not been quantified. This study demonstrates that patient-centeredness is a measurable ability of pediatricians that increases family trust.

This investigation tested the patient-centered Primary Provider Theory of Patient Satisfaction across gender in national random samples of emergency patients. Using multigroup structural equation modeling, the results supported the model's robustness. Physician service, waiting time, and nursing satisfaction explained 48%, 41%, and 11% of overall satisfaction plus 92% and 93% of female and male satisfaction, respectively.

The Primary Provider Theory holds that patient satisfaction occurs at the nexus of provider power and patient expectations. More specifically, patient satisfaction is principally the function of an underlying network of interrelated satisfaction constructs--satisfaction with the primary provider, waiting for the provider, and satisfaction with the provider's assistant(s). Hierarchically linked to patient-centered expectations of provider value, the Theory specifies that primary providers offer the greatest clinical utility to patients.
