Palm fat has uniquely optimal melting characteristics that are difficult to replace in products such as baked goods and chocolate-based items. This study investigates the efficacy of using Pickering emulsions derived from Swiss plant oils and their micromilled press cakes. Emulsification was carried out at both the lab and pilot scales using sunflower- and rapeseed-based recipes, with and without additional surfactants, for both oil-in-water and water-in-oil emulsions.

For this purpose, in a first step, the whole wheat kernels were pre-treated using cold needle perforation. The pore size was evaluated by scanning electron microscopy and the accessibility of enzymes and microorganisms determined using fluorescent markers in the size range of enzymes (5 nm) and microorganisms (10 μm), and fluorescent microscopy.
View Article and Find Full Text PDFFlexible flatfeet in children are still treated with orthotics and operative procedures by some physicians who believe such treatment will prevent disability in adult life. Opponents of treatment cite military studies that show flexible flatfeet are not a source of disability in soldiers. Because no study of adult civilians relating arch configuration and pain have been reported, this study was undertaken.
View Article and Find Full Text PDFFlatfoot may be classified as pathologic or physiologic. Pathologic flatfoot is often characterized by stiffness of the foot, causes disability, and requires treatment. Physiologic flatfoot is a normal variation; it causes no disability and tends to improve with time.

The treated cases characterized their remembrance of the experience through a subjective report questionnaire. Those who wore shoe modifications during childhood showed lower self-esteem than controls (p < 0.

The clubfeet were managed by a regimen including initial stretching casts, posteromedial release, and postoperative splinting at night. The mean age at the time of surgery was 7.

Occlusion pressures were measured by Doppler, and tourniquet pressures were set 50 mm above this value. Of 29 cases, 86% were determined to provide adequate hemostasis throughout the procedure.

Rotational profiles and sprinting foot-progression angles were compared in 50 high school sprinters and 50 high school controls. Sprinters had a significantly lower (p < 0.

In 95 children with amyoplasia-type arthrogryposis multiplex congenita, 40 hip dislocations in 26 patients were found. In 16 of these 26 patients, bilateral (nine patients) and unilateral (seven patients) dislocations were reduced by a medial-approach open reduction. The mean age at the time of surgery was 8.
View Article and Find Full Text PDFFive new cases of femoral neck stress fracture in children and adolescents are added to seven cases in the literature. All patients were involved in increased repetitive activity without evidence of violent trauma. Most fractures healed uneventfully with appropriate treatment.

We report two cases, each of which sustained two separate periarticular fractures from overzealous manipulation for knee contracture. The four fractures reported in this study involve one normal child sustaining asynchronous ipsilateral distal femoral and proximal tibial fractures and a child with the diagnosis of amyoplasia sustaining bilateral proximal tibial fractures. The child with knee contracture must be treated carefully and not exposed to overzealous physiotherapy or manipulation.

There were 10 girls and 4 boys with a mean age of 7 years and 9 months. All had functional and cosmetic disability before operation.

The mean age at the time of the operation was twelve months (range, two to sixty-three months), and the mean duration of follow-up was six years (range, two to thirteen years). Avascular necrosis was evident preoperatively in two hips (3 per cent) and was noted postoperatively in another seven hips (11 per cent).

Knee angle and intermalleolar (IM) or intercondylar (IC) distance were measured in 196 white children aged 6 months to 11 years to establish normal limits of tibiofemoral angle. Children were maximally bowlegged at age 6 months and progressed toward approximately neutral knee angles (0 degree) by age 18 months. Greatest mean knock knee of 8 degrees was observed at age 4 years, followed by a gradual decrease to a mean of < 6 degrees at 11 years.

Pseudoparalysis may be the only manifestation of septic arthritis of the hip in the neonate; a limp is often the presenting complaint in Perthes' disease, and scoliosis may be the primary manifestation of a herniated disc in an adolescent.

A retrospective analysis was done of 52 rotational tibial osteotomies (RTOs) performed on 35 patients with severe idiopathic tibial torsion. Thirty-nine osteotomies were performed at the proximal or midtibial level. Thirteen were performed at the distal tibial level with a technique previously described by one of the authors.

Follow-up of 24-168 months (mean 60 months) was obtained in 108 hips in 98 patients.
