Fever, confusion, acute kidney injury: is this atypical neuroleptic malignant syndrome following polypharmacy with clozapine and risperidone?

Objective: Clozapine is the gold-standard antipsychotic medication for treatment-refractory schizophrenia (TRS). However, one potentially lethal side effect of clozapine, as with other antipsychotics, is neuroleptic malignant syndrome (NMS) which could present differently in clozapine therapy. 'Atypical NMS' is a recognised variant of NMS with less rigidity and delayed elevation of creatine kinase; this variant is associated with clozapine.

Glutamate receptors play a major role in neural cell plasticity, growth, and maturation. The degree to which ionotropic glutamate receptors (iGluR) conduct current is dependent on binding of extracellular ligands, of which glutamate is the native agonist. Although the glutamate binding site of the GluR2 class of amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) iGluR has been structurally characterized, the allosteric sites attributed to neurosteroid binding have yet to be localized.

[Postoperative complications and mortality in peptic ulcer of the duodenum and stomach].

During 19 years (from 1974 to 1992) 5205 operations for complicated ulcer disease of duodenum (4201 observation), stomach (915) and of double localization (89) were conducted. In 585 of 5205 patients complications occurred: of operative wound healing-in 17.4%, in the region of intervention--in 56%, dysfunction of other organs and systems--in 26.

[The first case of Hb Camden carriage in Russia].

Structural hemoglobinopathy due to carriage of Hb Camden was diagnosed in a male patient and his mother. Anomalous Hb was detected after a detailed analysis of the clinical evidence, blood smears microscopy, marrow examination, data obtained by a complex of methods to identify unstable Hb. It was defined as Hb Camden beta 131 GLN-->GLU.

[Simultaneous operations in the elective surgery of abdominal viscera].

Plasma desorption mass spectrometry of haemoglobin tryptic peptides for the characterization of a Hungarian alpha-chain variant.

S-Aminoethylated-alpha A and -beta A globin tryptic peptides separated by reversed-phase high-performance liquid chromatography have been analysed by plasma desorption mass spectrometry. Almost all the expected alpha A and beta A tryptic fragments were tentatively assigned relative to the known globin chain sequences based on the molecular weight obtained by plasma desorption mass spectrometric analysis of the purified peptides. The application of plasma desorption mass spectrometry for structure elucidation of a haemoglobin alpha-chain variant revealed the first case of Hb Hasharon in Hungary.

[The principles of the surgical treatment of peptic ulcer of the anastomosis and jejunum].

The authors disposing of a great number of observations of patients operated upon for peptic ulcer of the anastomosis (570), give an account of main principles of performing surgical procedures. They are: the detection and elimination of the causes of the anastomosis ulcer, wider use of truncal vagotomy, and for gastro-intestino-colonic fistula a less traumatic intervention should be chosen--the disconnection of the fistula and suturing the colon. The ways of reduction of postoperative lethality of the patients with this severe disease are shown.

[The second family with hemoglobin Dagestan (alpha 60 Lys----Glu)].

Data have been presented on a new hemoglobin detected in a family of the Lezghin population in the Daghestan ASSR. The structural analysis of this hemoglobin has shown its correspondence to hemoglobin "Daghestan" that was previously detected among the population living in the Tabasaransk region. A detailed genetic analysis has shown a possibility of the gene drift.
