Background: Acute ischemic stroke (AIS) due to anterior circulation tandem lesion (TL) remains a technical and clinical challenge for endovascular treatment (EVT). Conflicting results from observational studies and missing evidence from the randomized trials led us to report a recent real-world multicenter clinical experience and evaluate possible predictors of good outcome after EVT.

Methods: We analyzed all AIS patients with TL enrolled in the prospective national study METRICS (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers).

Quality and mutagenicity of water and sediment of the streams impacted by the former uranium mine area Olší-Drahonín (Czech Republic).

The water quality research performed in the years 2003-2010 demonstrated an impact of the mine water pumped from the closed Olší uranium mine and discharged from the mine water treatment plant (MWTP) and groundwater from springs in the area on the water quality of the Hadůvka stream. The water ecosystems of the lower part of the Hadůvka stream are impacted mainly by water originated from the springs located in the stream valley and drained syenit subsoil, naturally rich in uranium. Those inflows caused a very high concentration of uranium measured in the water of the stream, which exceeds the given limit value.

Ecological risk assessment of lead contamination at rifle and pistol ranges using techniques to account for site characteristics.

Spent ammunition at outdoor rifle and pistol (RP) firing ranges creates a characteristic pattern of contamination, whereby small areas surrounding backstop berms exhibit extremely high soil lead (Pb) concentrations. We characterized sources, pathways and uncertainty in contaminant accumulation for receptors at two RP ranges in southeastern Ontario. Based on these results, we performed risk calculations using kriging to estimate risk across "worst-case" species foraging ranges.

[Unusual case of the long term presence of esophageal foreign body (non-radiopaque coin) in children].

The relative influence of distant and local (DEW-line) PCB sources in the Canadian Arctic.

Soil PCB contamination has been delineated at 18 of 21 Distant Early Warning Line (DEW-line) stations being cleaned up by the Canadian Department of National Defence (DND). As a result, detailed surface soil delineation data has been reported for contamination exceeding 1 microg/g (dw total Aroclor), which is the remedial criteria for PCB contaminated soil under the DEW-line cleanup project. The results of this delineation work has allowed us to estimate the mass of PCB contained in surface soil at these sites and to quantify the DEW-line as a source of PCBs to both local and Arctic wide contamination.

[Rare cases of neurofibroina in the head and neck].

Four cases of neurofibroma rare localized in parapharyngeal space and throat, in external ear and larynx are described. Three of them were isolated tumors and in one case--tumors in external ears developed in course of general Recklinghausen disease. Two cases of neurofibroma in parapharyngeal space had different clinical, CT and microscopic picture.

[Ocular and orbital complications of paranasal sinusitis].

Purpose: The task of the research was epidemiological and clinical analyses of those patients.

Material And Methods: In the period of 1976-200 147 patients with complications of perinasal sinusitis were treated. As numerous classifications of paranasal sinusitis complications exist, the analysis of the material was based on the modified classification proposed by the authors.

[Otologic signs and symptoms as first manifestations of Wegener's granulomatosis with very severe clinical course--review of the literature and case report].

[Long term administration of itraconazole with surgical treatment in fungal and bacterial infections of the paranasal sinuses].

A case of chronic paranasal sinuses with recurrent polyposis caused by miscellaneous infection--fungal (Aspergillus, Candida) and bacterial (Staphylococcus aureus, Enterobacter, Streptococcus) is described. The patient underwent 5 times surgical treatment (polypectomies, sinus operations). Good result was achieved after 2-years application of itraconazole and local Amphotericin B.

[Unusual case of the ear cholesteatoma].

The authors present a case of temporal and occipital bone extensive destruction with epidural abscess of posterior cranial cavity and abscess of occipito-nuchal region created by giant cholesteatoma of the middle ear. There has been discussed a diagnosing troubles as well as performed surgical treatment.

[Subperiosteal orbital hematoma as a sinusitis complication].

A case of subperiosteal orbital hematoma as a sinusitis complication--very rarely described in otolaryngological literature--is presented. Taking into account clinical examination and CT there was diagnosed subperiosteal orbital abscess but during surgery there was found hematoma. There has been discussed a clinical picture and a likely pathological mechanism of the complication as well as the performed surgical treatment.

[Teratoma in the middle ear and throat of an infant].

The authors present a case of an 8-moth-old female infant treated for prolonged otitis media. The inflammatory process was connected with teratoma of tympanum and throat. The authors discuss presumable connection of the both masses.

[An unusual case of the otogenic purulent thrombosis of the sigmoid sinus and internal jugular vein].

The authors present a case of 51 years old female suffering from the purulent thrombophlebitis of the sigmoid sinus and internal jugular vein. The disease developed in the course of chronic otitis media. During first 2 weeks she was treated with antibiotics because of the septic fever without significant otogenic symptoms.

[Adenolymphoma (Warthin's tumor) of the larynx: coexistence with the bilateral laryngocele. Contribution to differential diagnosis with oncocytic papillary cystadenoma].

Adenolymphoma is a benign monomorphic lesion composed of epithelial tissue. Usually it is found in parotid gland; however, its laryngeal occurrence was rarely described. Extraparotid Warthin's tumour as scanty lymphatic tissue.

Accelerated phase of chronic myeloid leukemia presenting with hypercalcemia and a mediastinal mass.

A patient with chronic myeloid leukemia developed hypercalcemia as a presenting sign of the accelerated phase of the disease. Ultrasound of the neck showed a large hypodense mass connected to the thyroid gland, which was thought to be a parathyroid tumor and the cause of the hypercalcemia. Histology of the surgically removed mass revealed a chloroma.

[Thyroid neoplasm with symptoms of carotid body tumor].

The authors present a case of thyroid cancer manifestating itself as carotid body tumor. Fine needle biopsy performed twice and an arteriography suggested this diagnosis.

[Head and neck lymphomas in the material of the ENT department of Municipal Hospital in Gdynia in 1977-1996].

The authors present data relating to patients with malignant lymphoma of the head and neck diagnosed at the ENT Department, Municipal Hospital in Gdynia, during the years 1977-1996. The material is analysed in relation to the sex and age of the patient and the localisation of tumour. Some cases are presented in detail depending on localisation or difficulties in diagnosis (lymphoma of the larynx and ethmoidal sinus).

Delayed abortion among teenagers: can a population at risk be identified?

Although only a small proportion of abortions performed in the United States are second-trimester abortions, teenagers obtain a large percentage of them. Yet the literature on abortion seekers is remarkably lacking in a discussion of who the pregnant teenagers who seek abortions are, particularly those who seek second-trimester abortions. We report a study of 97 adolescents who turned for help to a public abortion clinic in Missouri during 1989.

The effect of desmopressin on massive gastrointestinal bleeding in hereditary telangiectasia unresponsive to treatment with cryoprecipitate.

A patient with hereditary hemorrhagic telangiectasia and reduced levels of von Willebrand factor had severe recurrent gastrointestinal bleeding. Treatment with either desmopressin acetate or cryoprecipitate normalized both bleeding time and kaolin-activated partial thromboplastin time. Hematochezia (five episodes) continued, however, for a week despite daily treatment with cryoprecipitate, whereas treatment with desmopressin at the onset of three later episodes of hematochezia resulted in the prompt cessation of bleeding.
