Screening for carcinoma in situ of the contralateral testis in patients with germinal testicular cancer.

Two hundred and fifty biopsy specimens from the contralateral testis in patients with unilateral germinal testicular cancer were analysed by light microscopy for carcinoma-in-situ changes. Changes were found in 13 (5.2%) patients.

Effects of low-dose ketamine and thiopentone on cardiac performance and myocardial oxygen balance in high-risk patients.

Induction of anaesthesia may pose a significant hazard to patients with critical cardiovascular status. Ketamine has been advocated as the drug of choice for maintaining cardiovascular performance during induction of anaesthesia in severely ill surgical patients. The purpose of this study was to compare the relative changes in the haemodynamic effects of ketamine and thiopentone during the first 30 min of anaesthesia induction measured by thoracic impedance cardiography.

Vitamin A palmitate decreases intravenous glucose tolerance in man.

We tested retinyl palmitate for in vivo effects in man and in vitro effects on the IM-9 lymphocyte insulin receptor. Intravenous glucose tolerance tests (IVGTT) with 25 g glucose were performed on 10 healthy subjects before and after two intramuscular injections of retinyl palmitate (25,000 IU) 18 hours apart. In 9 of 10 subjects, glucose disposition was impaired after treatment with retinyl palmitate.

Angiolymphoid hyperplasia with eosinophilia (Kimura's disease). A case with dermal lesions in the knee region and a popliteal arteriovenous fistula.

A 28-year-old man with slight blood eosinophilia had multiple dermal lesions of angiolymphoid hyperplasia with eosinophilia in the right knee region. A fistula between the popliteal artery and vein was demonstrated by arteriography. Between the tortuous blood vessels composing this shunt, an inflammatory reaction similar to that found in the superficial lesions was seen.

Personality in unipolar and bipolar manic-malancholic patients.

By use of standardized case-recording criteria (MULTI-CLAD) manic-melancholic patients were classified as unipolar (n = 13) and bipolar (n = 23). Personality patterns are evaluated when patients were in neutral mood (defined by low symptom rating scale scores). Results showed that the similarities in the personality patterns of unipolars and bipolars were much pronounced than the differences.

A modern burns unit.

In the year 1961 a special Burns Unit was opened in the old Municipal Hospital. As it was decided to build the new Hvidovre Hospital it was planned also to move the Burns Unit to this large University hospital which contained all other specialities. The department for plastic surgery and the Burns Unit consists of 35 beds, 7 of which are part of the intensive care unit.

Incidence of carcinoma in situ of germ cells in contralateral testis of men with testicular tumours.

Biopsy specimens from the contralateral testicle in 50 consecutive patients with germinal testicular cancer were examined for carcinoma in situ. Three out of 21 men with seminomas and one out of 29 with other types of germinal cancer (8%) had carcinoma in situ in the contralateral testicle without any clinical signs. One of these men developed early invasive germ-cell cancer 46 months after carcinoma in situ was first diagnosed.

Acute excision or exposure treatment? Results of a randomized controlled clinical trial.

An evaluation of a total of 2,460 patients treated with exposure during the period 1962-1971, marked that the exposure method showed satisfactory results, but also that there are clear upper limits for the possibilities of this method. The approach of treatment was reconsidered and consequently very severely burned patients were excised acutely in the years 1972-1975. It was proved plausible, permissable and practically possible.

Cardiac function and hypercarbia.

In 12 patients with heart disease, hypercarbia was induced for carotid endarterectomy. Anesthesia was maintained with nitrous oxide in oxygen and methoxyflurane. In addition to intra-arterial measurements of blood pressure, cardiac output, systolic time intervals (STI), and pressure time indices (PTI) were determined in order to assess cardiovascular responses in these patients.

The practical significance of nortriptyline plasma control. A prospective evaluation under routine conditions in endogenous depression.

The problem of the advantage of using nortriptyline (NT) plasma level during treatment for endogenous depression has been approached. In an ordinary psychiatric department, 34 patients had their NT level checked the second week of treatment and their dosage subsequently adjusted, if it was outside the recommended therapeutic plasma range (50--150 ng/ml). A cautious dose policy led to low plasma levels followed by dose increase in about 40% of the patients.

Bow hunter's stroke.

A case of suspected brain stem infarction (modified Wallenberg syndrome) due to vertebral artery injury in a bow hunter is presented. An anatomical review is presented to explain the neurological involvement, and recommendations are given for avoiding this syndrome.
