Background: Medically intractable ascites causes substantial distress in patients with palliative disease. Tunneled peritoneal catheters have been established as a feasible treatment option allowing patient-controlled paracentesis in a homecare setting. However, while a range of complications is associated with these drainages, risk factors for complications have not been identified so far.

Article Synopsis
  • A 70-year-old Caucasian man with advanced colorectal cancer developed a complete obstruction in his small intestine six months after getting a diverting ileostomy.
  • He was not eligible for surgery due to previous tumor perforation, so doctors used an endoscopic ultrasound-guided technique to create a new passage in the intestines.
  • The procedure was successful, allowing the patient to resume oral nutrition the following day and leave the hospital in better health after three days.

Article Synopsis
  • The study explores the real-world treatment patterns for inflammatory bowel diseases (IBD) at a large referral center, highlighting the challenges in decision-making due to the variety of therapies available.
  • A total of 502 patients with Crohn's disease and ulcerative colitis were analyzed, showing that infliximab remains the most commonly used first-line therapy, while newer options like ustekinumab and vedolizumab are gaining traction.
  • The findings indicate that remission rates decline with subsequent lines of therapy, pointing to the need for better biomarkers to guide treatment choices in IBD.

Article Synopsis
  • * The research finds significant correlations in osteochondral parameters depending on specific locations within the joint, showing how varus malalignment affects less loaded areas while leaving overloaded areas less responsive.
  • * The study suggests that understanding axial alignment is crucial when assessing OA in clinical settings and supports the idea of therapeutic approaches like load redistribution and weight loss for managing varus knee OA.

Articular cartilage damage occurring during early osteoarthritis (OA) is a key event marking the development of the disease. Here, we modeled early human OA by gathering detailed spatiotemporal data from surgically induced knee OA development in sheep. We identified a specific topographical pattern of osteochondral changes instructed by a defined meniscal injury, showing that both cartilage and subchondral bone degeneration are initiated from the region adjacent to the damage.
