The Omicron variant has been reported to present with milder disease compared with Delta, although this may be due to immunity from vaccination and prior exposure. Predictors of severity with recent strains have not been well characterized. We retrospectively examined consecutive cases of moderate-to-severe COVID-19 (defined as requiring supplemental oxygenation, intensive care or mortality) admitted to seven tertiary hospitals across Singapore in April 2023.

Introduction: We previously reported that endoscopic repair of a Type 1 Laryngeal Cleft (LC1) or Deep Interarytenoid Groove (DIG) improves swallowing function postoperatively. However, caregivers often ask about the timeline to resolution of the need for thickening. This study re-examines this cohort to answer this important caregiver-centered question.

Paddy plants (Oryza sativa) contaminated with metals could be detrimental to human health if the concentrations of metals exceed the permissible limit. Thus, this study aims to assess the risk of the concentrations of As, Se, Cu, Cr, Co, and Ni and their distributions in various parts (roots, stems, leaves, and grains) of paddy plants collected from Sekinchan, Malaysia. Both soil and plant samples were digested according to the United States Environmental Protection Agency (USEPA) Method 3050B and the metal concentrations were determined by the Inductively Coupled Plasma-Mass Spectrometry (ICP-MS).

Objective: Most studies observing an association between depressive symptoms following lung transplantation and mortality are limited to depressive symptom measurement at a single time point, unrelated to allograft function. We aimed to test the association of depressive symptoms over multiple assessments with allograft dysfunction and with mortality.

Methods: We assessed depressive symptoms before and serially up to 3 years after lung transplantation in lung transplant recipients.

Background: The rising incidence of chronic diseases is a growing concern, especially in Singapore, which is one of the high-income countries with the highest prevalence of diabetes. Interventions that promote healthy lifestyle behavior changes have been proven to be effective in reducing the progression of prediabetes to diabetes, but their in-person delivery may not be feasible on a large scale. Novel technologies such as conversational agents are a potential alternative for delivering behavioral interventions that promote healthy lifestyle behavior changes to the public.

Post-operative delirium after lung transplantation is common. Its associations with health-related quality of life (HRQL), depression, and mortality remains unknown. In 236 lung transplant recipients, HRQL and depressive symptoms were assessed as part of a structured survey battery before and after transplantation.

Disability, depressive symptoms, and impaired health-related quality of life (HRQL) are common among patients with life-threatening respiratory compromise. We sought to determine if primary graft dysfunction (PGD), a syndrome of acute lung injury, attenuates improvements in patient-reported outcomes after transplantation. In a single-center prospective cohort, we assessed disability, depressive symptoms, and HRQL before and at 3- to 6-month intervals after lung transplantation.

Background: Lung transplantation and related medications are associated with pathobiological changes that can induce frailty, a state of decreased physiological reserve. Causes of persistent or emergent frailty after lung transplantation, and whether such transplant-related frailty is associated with key outcomes, are unknown.

Methods: Frailty and health-related quality of life (HRQL) were prospectively measured repeatedly for up to 3 years after lung transplantation.

Background: While lung transplantation (LTx) improves health-related quality of life (HRQL) in cystic fibrosis (CF), the determinants of this improvement are unknown. In other populations, frailty-a syndrome of vulnerability to physiologic stressors-is associated with disability and poor HRQL. We hypothesized that improvements in frailty would be associated with improved disability and HRQL in adults with CF undergoing LTx.

Article Synopsis
  • * A scoping review of 20 studies with over 4,000 participants revealed key information needs in areas such as disease understanding, patient care, healthcare services, and caregiver self-care, with a particular focus on healthcare service-related information.
  • * The findings indicate that while existing information resources are abundant, there's a need for more emphasis on healthcare services, and most research comes from high-income countries, primarily addressing non-professional caregivers' needs based on the severity of dementia.

Background: Dementia is a debilitating disease that can lead to major changes in a patient's behaviour and function. It is important to educate both patients with dementia and their non-professional caregivers about the disease. Yet, currently available sources do not seem to be effective for patients and caregivers, who report a need for more information and guidance.

Aim: Diabetes in young adulthood has been associated with poor outcomes. Self-management is fundamental to good diabetes care, and self-management interventions have been found to improve outcomes in older adults. We performed a systematic review and meta-analysis to assess the effectiveness of self-management interventions in young adults (aged 15-39 years) with type 1 or type 2 diabetes.

Article Synopsis
  • The immune system typically fights off pathogens while ignoring the body’s own signals, but during lung transplantation, issues can arise with endogenous mitochondrial DNA (mtDNA) contributing to organ damage.
  • Researchers found that mtDNA is higher in lung transplant cases with primary graft dysfunction (PGD) and triggers inflammation through TLR9 receptors, leading to harmful neutrophil extracellular trap (NET) formation.
  • Therapy targeting DNaseI could offer a dual benefit in PGD by neutralizing the triggering mtDNA and breaking down the damaging NETs, ultimately helping to reduce lung injury.

Background: Frailty is common in adults with advanced lung disease and is associated with death before and after lung transplantation. We aimed to determine whether frailty changes from before to after the lung transplant.

Methods: In a single-center, prospective cohort study among adults undergoing lung transplantation from 2010 to 2017, we assessed frailty by the Short Physical Performance Battery (SPPB; higher scores reflect less frailty) and Fried Frailty Phenotype (FFP; higher scores reflect greater frailty) before and repeatedly up to 36 months after transplant.

Rationale: Although lung transplantation aims to improve health-related quality of life (HRQL), existing instruments fail to include health domains considered important in this population.

Objectives: We aimed to develop a comprehensive lung transplant-specific instrument to address this shortcoming.

Methods: We developed a pool of 126 candidate items addressing domains previously identified as important by lung transplant recipients.

Frailty is associated with increased mortality among lung transplant candidates. We sought to determine the association between frailty, as measured by the Short Physical Performance Battery (SPPB), and mortality after lung transplantation. In a multicenter prospective cohort study of adults who underwent lung transplantation, preoperative frailty was assessed with the SPPB (n = 318) and, in a secondary analysis, the Fried Frailty Phenotype (FFP; n = 299).
