Article Synopsis
  • - The study aimed to evaluate the outcomes of severe hypoglycaemia (SH) and diabetic ketoacidosis (DKA) in adults with type 1 (T1D) and type 2 diabetes (T2D) who visited the emergency department, analyzing their medical records between January 2019 and June 2023.
  • - It found that DKA was more common in T1D patients, while SH mainly affected T2D sufferers, with T2D patients showing higher mortality rates and comorbidities compared to T1D patients after experiencing SH and DKA events.
  • - The research concluded that SH and DKA cases in T2D individuals are associated with a poor prognosis, especially for those with

Article Synopsis
  • The study examined severe hypoglycaemia (SH) cases treated at a hospital in Sheffield, focusing on clinical characteristics, risk factors, and mortality outcomes over 44 months.
  • A total of 619 SH episodes were recorded in 506 individuals, most of whom had diabetes, but a significant portion were non-diabetic.
  • The research found higher mortality rates in non-diabetic and type 2 diabetes patients compared to type 1 diabetes patients, with non-cardiovascular causes being the leading reasons for death, emphasizing the role of multimorbidity in increasing mortality risk.

COVID-19 has had a significant impact on internal medicine training in the UK. Many unprecedented changes have been made to prioritise the care of affected patients. The medical workforce was re-shaped, training programmes were disrupted, Membership of the Royal College of Physicians (MRCP) examinations were cancelled and their format changed on re-commencement, teaching programmes were suspended and delivery methods amended, out of programme (OOP) opportunities deferred, non-COVID related research halted, trainee progression impacted and trainee mental health and wellbeing suffered.

Aims/hypothesis: Few studies examine the association between age at diagnosis and subsequent complications from type 2 diabetes. This paper aims to summarise the risk of mortality, macrovascular complications and microvascular complications associated with age at diagnosis of type 2 diabetes.

Methods: Data were sourced from MEDLINE and All EBM (Evidence Based Medicine) databases from inception to July 2018.

Background This study aimed to investigate the relationship between body mass index (BMI) and sufficient chest compression depth (CCD) in obese patients by a mathematical model. Methods and Results This retrospective analysis was performed with chest computed tomography images conducted between 2006 and 2018. We classified the selected individuals into underweight (<18.

Background: Current guidelines recommended that chest compression depths during car-diopulmonary resuscitation (CPR) should be at least one-fifth of the external chest ante-riorposterior (AP) diameter. The chest AP diameter increases because of dorsal kyphosis, senile emphysema, and poor lung compliance associated with aging. This study aimed to compare the proportion of the heart compressed by chest compression (based on the ejection fraction [EF]) in geriatric and nongeriatric patients.

Background: Abdominal fatty tissue deposition in obese individuals could alter the proper hand position for chest compression during cardiopulmonary resuscitation, similar to that in pregnant women. This study aimed to identify the difference in body mass index between obese and normal weight individuals by measuring the optimal point of maximal left ventricular diameter (OP), using computed tomography (CT).

Methods: We performed a retrospective analysis of chest CT scans between January 2012 and August 2016 and measured the sternal length and OP and estimated the ratio of OP to that individual sternal length.

Early evidence has shown that Accountable Care Organizations (ACOs) have achieved some success in improving the quality of care and reducing Medicare costs. However, it has been argued that the ACO rewarding model may disproportionately affect relatively low-spending (LS; considered as efficient) organizations that have fewer options to cut unnecessary services compared with high-spending (HS; inefficient) organizations. We conducted a cross-sectional retrospective study to compare ACO financial and quality of care performance between HS-ACO and LS-ACO.

A 48-year-old woman presented to the Accident and Emergency department with a 4 month history of headaches, nausea and dizziness. She was found to have severe hypertension and hypokalaemia. Extensive investigations did not find any secondary cause for hypertension.

The purpose of this study was to assess the incidence of spontaneous renal artery dissection (SRAD) as a cause of acute renal infarction, and to evaluate the clinical and multidetector computed tomography (MDCT) findings of SRAD. From November 2011 to January 2014, 35 patients who were diagnosed with acute renal infarction by MDCT were included. We analyzed the 35 MDCT data sets and medical records retrospectively, and compared clinical and imaging features of SRAD with an embolism, using Fisher's exact test and the Mann-Whitney test.

Background: In the UK, the care of young people with diabetes has focused predominantly on type 1 diabetes (T1D). However, young-onset T2D has become increasingly prevalent. At present, it is unclear which type of diabetes represents the more adverse phenotype to develop complications.

Background/aims: To investigate semiquantitative analyses based on amount and morphology of iodized oil uptake about non-enhanced C-arm cone-beam CT(C-arm CT) immediately following hepatic chemoembolization of HCC.

Methodology: We retrospectively reviewed 40 C-arm CT images taken immediately following chemoembolization in 29 consecutive patients who underwent hepatic chemoembolization for HCC lesions(n=97). Two radiologists scored iodized oil uptake in the index tumours from Grade 0(complete) to 4(invisible).

Background/aims: Because of the similarities in the clinical presentations of Crohn's disease (CD) and intestinal tuberculosis (ITB), differential diagnosis is critical. Mesenteric adipose tissue hypertrophy and creeping fat are characteristic features of CD. The purpose of this study was to assess the usefulness of visceral fat for the differential diagnosis of CD and ITB.

Type 2 diabetes mellitus (a disease normally appears in the post 40 age group people) now seems to emerging in young adults at the level of global epidemic driven by the increasing burden of obesity. Evidence is accumulating to suggest that this young diabetic cohort is an aggressive phenotype which leads to the premature development of complications that not only have impact on the quality of life but also unfavourably influence the long term outcome, raising the possibility of a serious public health challenge in the next few decades. This chapter reviews the current understanding of the phenomenon of T2DM in the young adults and discusses the clinical challenges in managing this high risk group.

Purpose: To investigate the clinical benefits of F18-fluorodeoxyglucose-positron emission tomography and computed tomography ((18)F-FDG-PET/CT) over multi-detector row CT (MDCT) in preoperative staging of gastric cancer.

Methods: FDG-PET/CT and MDCT were performed on 78 patients with gastric cancer pathologically diagnosed by endoscopy. The accuracy of radiologic staging retrospectively was compared to pathologic result after curative resection.

Electrolyzed reduced water (ERW), functional water, has various beneficial effects via antioxidant mechanism in vivo and in vitro. However there is no study about beneficial effects of ERW bathing. This study aimed to determine the effect of ERW bathing on the UVB-induced skin injury in hairless mice.
