On the grounds of own and obtained from literature data the authors assert that under conditions of out-patient departments detection of subjects with high risk of sudden death among cardiological patients remains unsatisfactory. System of organizational diagnostic and therapeutic measures is suggested with the view to improve effectiveness of sudden death prevention in patients with cardiovascular diseases.

It was found that the dynamics of mortality from ischemic heart disease showed a negative tendency among the population of Kiev as compared with urban population of other cities of Ukraine. An increase was noted of mortality among the workable population of Kiev and also of the non-hospital mortality.

An analysis is presented of the status of arterial hypertension prophylaxis in the Ukraine. Drawbacks in solution of this problem are discussed: diagnosis is incomplete, prophylactic management of hypertensive disease is inadequate. Ways and means of improving the diagnosis and prophylaxis of hypertensive disease in the Ukraine including better remuneration of the medical personnel and interests of the population in preserving their health are discussed.

Examination of 4185 men aged 30 to 59 years, carried out within the framework of investigations into multifactorial prevention of coronary heart disease enabled one to establish that the number of tobacco-smokers among that group men amounts to 45.1%. The percentage of tobacco-smokers appreciably decreases with age.

The method of dietetic interview was used in a strictly standardized epidemiological investigation of a random sample of males aged from 30 to 59 years (n-2537) in Kiev. A dependence has been revealed between the use of alcohol and disorders in the character of nutrition unfavourable in respect to coronary heart disease. A relationship has been established between the alcohol use and the incidence rate of such risk factors of coronary heart disease as hyperlipoproteinemia, excessive body mass, smoking, and the incidence of myocardial infarction.

EIA was used to demonstrate that the development of acute ischemia of the heart muscle progressing to necrosis is accompanied by an increase of the concentration of cytochrome c and antibodies against cytochrome c in the blood serum. As regards its specificity and sensitivity, the alteration in the antibody concentration is comparable with that in the myoglobin concentration but it is marked 1 to 3 hours earlier. The high level of antibodies against cytochrome c remaining unchanged for 2 to 3 days in myocardial infarction patients attests to the possibility of a complicated, often relapsing course of the disease.

The use of up-to-date diagnostic methods and the estimation of the extent of myocardial lesion in patients with intermediate coronary conditions (serial assays of CPK and MV-CPK activity with subsequent computation of the weight of affected myocardium, ECTG and myocardial 99mTc-Sn-pyrophosphate scintigraphy using a gamma-counter) demonstrated: the presence of necrotic foci in patients with acute focal myocardial dystrophy; a considerable variation in the weight of affected myocardium; and high diagnostic value of ECTG for the assessment of the extent of myocardial lesion in patients with intermediate coronary conditions. Objective criteria have been worked out for the differential diagnosis of various intermediate coronary conditions.

Dosaged restriction of coronary blood flow and its subsequent restoration were conducted in acute experiments on anesthesized dogs under conditions of closed chest and preserved natural respiration. The degree of ischemic damage to the heart muscle was determined by means of precardial ECG, the functional condition of the left ventricle was studied according to the data on intraventricular pressure and the rate of its changes with subsequent calculation of the contractility indices. The indices of general hemodynamics were also studied.
