A prospective study was done on the erectile function of 35 randomly-selected patients undergoing transurethral prostatectomy for benign prostatic hypertrophy. A preoperative sexual history was obtained, and the erectile function of each patient was recorded using a nocturnal penile tumescence monitor. Four to six weeks postoperatively, a repeat sexual history was taken and the erectile function was reassessed.

The clinical and angiographic features of 32 patients with cephalic fibromuscular dysplasia (FMD) are reported. All of our patients were women, 78% of whom were between 41 and 70 years of age. At examination, 18 (56%) patients had the sudden onset of focal ischemic neurologic deficits.

Review of the literature shows that long-term phenytoin therapy is associated with peripheral neuropathy and neuromuscular disorder that rarely can be symptomatic. The usual manifestations of the peripheral neuropathy are lower extremity areflexia, sensory deficits, and reduced conduction velocities. The risk for these peripheral nerve abnormalities is greater with increased serum levels or long duration of therapy (usually more than 5 years).

Our over-all results with intermittent clean catheterization in attaining continence and limiting the occurrence of significant infection while preserving renal function in children with neurologic bladder dysfunction have been gratifying and compare favorably to other series. The program has not only resulted in satisfactory continence in the majority but also it has been associated with marked improvements in personality, sociability and self-image in several of our older patients who are free from the odor, shame and discomfort of the incontinence for the first time in their lives. From our results and those of others it appears that intermittent clean catheterization is a safe, reversible and effective means of management of children with neurogenic bladders.

Epilepsy in adults.

Ann Neurol

January 1981

The past decade has seen advances in the management of patients with epilepsy. The development of practical long-term electroencephalographic techniques, with or without simultaneous video recording, has increased the accuracy of diagnosis of seizure types. The technique also provides clinicians and investigators with a method for establishing the clinical efficacy of antiepileptic drugs and determining their therapeutic serum concentrations.

A rare type of fibromuscular dysplasia of the common carotid artery, in the form of a septum, was observed in a patient suffering from recurrent cerebral infarctions. Angiography demonstrated growth of the lesion and documented the presence of an intraluminal thrombus attached to it. Comparison with reported cases of the more common "string of beads" lesion of fibromuscular dysplasia suggests that the risk for development of ischemic neurological problems is higher for this type of fibromuscular lesion.
