Background: Tuberculosis is a highly contagious disease, and there has been a rise in recent years of drug-resistant cases no longer responding to standard treatment. In order to address this threat and contain possible transmission of drug-resistant cases, some countries have taken strong action, including the compulsory detention of non-adherent drug-resistant patients. These measures have been strongly criticized by human rights advocates, and they raise the question of how to legally protect both citizens and the community.

What Is Known And Objective: Probiotic therapy has been shown to reduce morbidity and mortality of necrotizing enterocolitis (NEC) in premature infants in several international studies using various probiotic agents. The purpose of this study(*) is to describe our experience of using probiotic therapy in preventing NEC in infants with very low birth weight (VLBW) in a neonatal intensive care unit (NICU) and to evaluate whether our records provide evidence of effectiveness for probiotic therapy.
Methods: In a retrospective cohort study, the efficacy of probiotic therapy in preventing NEC in VLBW infants was investigated via chart review.
Two experiments were conducted with a commercial turkey company using a commercial egg injection system to investigate the effect of a dextrin-iodinated casein solution injected in ovo at 25 d of incubation on turkey poult hatchability, hatch weight, and growth (6 or 7 d posthatch). In experiment 1, a total of 3,900 turkey eggs (1,300 per group) were injected at 25 d of incubation with either 200 μL of a control (physiological saline) solution or a dextrin solution (18% maltodextrin and 10% potato starch dextrin) with 75 or 375 μg/mL of iodinated casein (DexIC75 or DexIC375, where Dex and IC refer to dextrin and iodinated casein, respectively). Two hundred poults from each group were neck-tagged, weighed (hatch weight), placed in a commercial turkey house within a single brooder ring, and weighed again (7 d posthatch).

This causes the entire volume to be weakly reflecting to a confocal read/write head. Superposition of two or three such gratings with slightly different k-vectors creates a track and layer structure that specialized servo detection optics can use to lock the focus to these deeply-buried tracks.

This optical Clos architecture is also well matched to a space-wavelength switch that arbitrarily permutes data streams between wavelength-division multiplexed channels on an array of fibers. Design: The study was a randomized, double-blinded, three period, crossover study.
Setting: The study was conducted at two sites: Park Nicollet Institute, International Diabetes Center, Minneapolis, MN, and Radiant Research, Inc.
Micrometer-sized reflection holograms can be written into a rapidly rotating homogeneous photopolymer disk at the focus of a high-numerical-aperture beam and its retroreflection to implement high-capacity multilayer digital data storage. This retroreflection is generated by an optical system with positive unity magnification to ensure passive alignment of the counterpropagating beam. Analysis reveals that the storage capacity and transfer rate of this bit-based holographic storage system compare favorably with traditional page-based systems but at a fraction of the system complexity and cost.
View Article and Find Full Text PDFObjective: To compare school performance at age 10 years in a cohort of extremely preterm children and term control subjects and to examine the impact of family composition and stability on performance.
Study Design: Prospective, longitudinal follow-up from birth to 10 years of age of a regional cohort of children born at 24 to 31 weeks of gestational age and sociodemographically matched term control subjects. Family composition, extent of parental care giving, and family moves were tracked sequentially.
Objective: In a large private tertiary care hospital we compared the two different approaches to group B streptococcal screening and intrapartum chemoprophylaxis suggested by The American College of Obstetricians and Gynecologists, the American Academy of Pediatrics, and the Centers for Disease Control and Prevention: risk factor-based protocol and culture-based protocol.
Study Design: A 2-year baseline period was followed by sequential prospective observational studies of the impacts of two different group B streptococcal management protocols, 3 years with the risk-based approach and 2 years with the culture-based approach of universal screening at 35 to 37 weeks' gestation.
Results: During the baseline period the rate of early-onset group B streptococcal infection was 1.
Today clinical information is used for a multitude of purposes beyond patient care documentation including quality review and improvement processes, allocation of resources, budgetary and long-term planning, productivity measurement, and justification to payers for services provided. Providers in perinatal medicine are faced with the challenge of finding methods to meet these information needs. Case examples of the different approaches to collecting and using obstetric and neonatal information are described.

We also examined the use of unwashed and older red blood cells. Red blood cells given to the study group were used without washing and until their expiration date (35 to 42 days).

A new approach to self-aligning unsupervised optical learning based on the competitive learning algorithm and adaptive holographic interconnectións is introduced. A volume hologram is used to diffract the light from an input spatial light modulator so that it focuses upon a custom winner-take-all very-large-scale-integrated circuit liquid-crystal spatial light modulator. The units that receive the most light switch to a reflective state, and the light reflected from the winning pixels interferes in the volume of the dynamic hologram with the phase conjugate of the input pattern in order to add an outer-product perturbation to the current holographic interconnection.

We are seeing an expansion beyond the traditional intrauterine-based short-term growth and nutrient retention rates toward a broader, and possibly life-long, range of outcomes. At day 1 after injection, there is increased aqueous humor fluorescence that reflects the physical disturbance to the eye following injection. From that time through 7 weeks, there is no evidence of any overt toxicity to the blood-retinal barrier.

Specific additives, proven to increase corneal endothelial permeability, were added to a purified oil, and placed into the vitreous of rabbits to assess their effects on the retina. Of the 305 centers responding to our survey, 78% had a database in use in 1989 and 15% planned to develop one in the future. Nurseries varied remarkably in the volume of data collected, the amount of time devoted to completing data collection forms, and the personnel involved in data collection.

The Bayley Mental Developmental Index at 6, 15, and 24 months and the McCarthy General Cognitive Index at 4 years were used to classify cognitive outcome for preterm children as normal (indices higher than 1 SD below the mean), mild-moderately delayed (indices between 1 and 2 SD below the mean), or severely delayed (indices > or = 2 SD below the mean). Classifications based on norms derived from the performance of the term control group were compared with those based on published standardized test scores.

One-dimensional arrays varying in size from 2 to 64 units and a 32 x 32 unit two-dimensional array have been fabricated and have demonstrated winner-take-all behavior. These devices have potential uses in optical implementations of correlators, associative memories, and unsupervised learning systems.

When bathed in basic tear solution alone, corneal swelling occurred at an average of 12 microns/h over a 3-hour period. Corneal swelling occurred at a rate of about 21 microns/h when the epithelial solution was switched from normal basic tear solution to an iso-osmotic K(+)-free basic tear solution.

The effects of intravitreal expansile gases, sulfur hexafluoride and octafluoropropane, as well as air and needle insertion alone were assessed by iris fluorescein angiography and blood-retinal barrier permeability determination. Iris angiography, at 3 days after injection, indicated no differences between experimental and paired control eyes. Vitreous fluorophotometry on day 1, day 4 and day 7 after gas or sham injection also showed no differences between paired control and experimental eyes regardless of which experimental condition was employed.

Both short- (1 week) and longer-term (7 week) studies have been made on a series of silicone oils to determine their effects on blood-retinal barrier permeability. About 25% of the vitreous humor was replaced with the test oil, and fluorophotometric determinations made after intravenous fluorescein at intervals after oil injection. The short-term studies revealed a uniform disturbance of the eye at 24 hours after oil injection that probably represents the physical disturbance of the eye during injection.
View Article and Find Full Text PDFThe morphologic changes in ciliary processes and the associated intraocular pressure (IOP) were observed in owl and squirrel monkeys after intravitreal (IVT) and intravenous (IV) injections of water soluble marijuana-derived material (MDM). The response in monkeys differed from that reported in rabbits wherein IV injection induced severe ciliary swelling and a significant decrease in IOP. Only moderate swelling occurs in monkey processes after IV injection of relatively high dose of MDM, and this change, which includes disruption of the basal lamina of the pigment epithelium, is not associated with a change in IOP.

In separate experiments, the vascular permeability of the inflamed eye was estimated by iris fluorescein angiography and by measuring protein influx into the aqueous humor. The effect of topical corticosteroid treatment upon blood flow and vascular permeability was also measured.
