Based on the experience gained with surgical treatment of 658 epileptic patients the authors discuss 7 cases of late diagnosis of gliomas of the temporal lobe. Cessation of the attacks after stereotaxic cryodestruction for 6 months to 6 years does not exclude that tumor may grow. The occurrence of new foci of neurological symptoms is prognostically unfavourable.

A total of 33 patients with brain tumors and 22 patients with temporal epilepsy were compared. The differential diagnostic signs at the initial stages were revealed. In tumors, the disease starts most commonly from paroxysms of the isolated aura or complex absence epilepsy types.

The course of pregnancy and parturition has been examined in 29 women who had been operated on for temporal epilepsy at the age of 13 to 29 years. The period of follow-up varied from 2 to 18 years. It has been established that pregnancy in these women is attended by a somewhat elevated predisposition to cerebral convulsions so that anticonvulsive therapy is indicated for such patients during this period.

Ninety-eight patients with long-term electrodes implanted into different structures of the brain were subjected to clinical and electrophysiological examinations. Motor phenomena activated by electrical stimulation of the brain were studied by radiotelemetry and stimulation myography. The threshold and amplitude of H-reflex was shown to be decreased during the interictal period.

Bitemporal epilepsy is characterized by complex hierarchical relationships between the contralateral temporal foci. On the basis of the clinical and electrophysiological examination of 61 patients with the clinical form of the disease which is discussed, the authors singled out definite characteristics the comprehensive evaluation of which allowed three possible types of subordination of the bitemporal foci that was studied to be identified. The authors believe that the singled out variants in the interrelationships of the foci of epileptic activity are not the only specific features in the organization of the epileptic system in bitemporal epilepsy.

Forty patients with temporal epilepsy were operated on with a stereotaxic technique. Eleven patients had a unilateral localization of the epileptic focus, and in 29 bitemporal foci were diagnosed. In evaluating the results of the treatment the dynamics of the epileptic fits, the peculiarities of changes in the mental status, and the degree of social adaptation were taken into consideration.
