Thirty-four years of experience gained with the treatment of patients suffering from rheumatoid arthritis made it possible to develop a complex of orthopedic treatment methods (conservative and surgical) based on the clinical, x-ray and morphological classification. These methods allow arresting inflammation in the joint, preventing destruction of the joint and formation of concordant and disconcordant deformities, correcting deformities, recovering function, the patients activity, returning ability to active life, to social useful work, and minimizing the patients' disability.

At the Ukrainian Centre for Hemophilia, from 1980, 123 patients with A and B hemophilia and involvement of the large joints have been treated. A programme for complex orthopedic-surgical treatment against the background of full-value antihemophilic therapy has been developed. Use of the given programme permitted to improve the results of treatment and prevent the development of severe arthropathy of the knee, ankle and elbow joints with preservation of their function in 50% of the patients.

With regard to the age of the patients, the localization of the fracture, the character of dislocation of the fragments, the manifestation of involutional and neurodystrophic changes at the proximal end of the femur and the presence of associated diseases the authors advise strictly differentiated approach in choosing the surgical method of treatment, especially in senile patients. It contributes to their early activization, improves the results of the treatment and reduces the mortality rate.
