Article Synopsis
  • The study compares the bleeding rates of EUS-fine-needle aspiration (FNA) and EUS-fine-needle biopsy (FNB) in patients with solid and cystic pancreatic masses.
  • Minor bleeding occurred more frequently when using FNB needles, particularly with cysts and lower platelet counts, while major bleeding was rare.
  • The findings suggest that while minor bleeding rates increase with the use of EUS-FNB, major bleeding remains a low-risk event across both needle types.

Background And Aims: Recently published criteria by 2019 Cirrhotic Cardiomyopathy Consortium set a lower threshold for reduced ejection fraction to diagnose systolic dysfunction in cirrhotic patients, and stress testing was replaced by echocardiography strain imaging. The criteria to diagnose diastolic dysfunction are in general concordant with the 2016 ASE/EACVI guidelines and differ considerably from the 2005 Montreal recommendations. We aimed to assess the prevalence of cirrhotic cardiomyopathy according to different diagnostic criteria.

Article Synopsis
  • Portal hypertension (PH) is a significant issue in liver cirrhosis, leading to serious complications like variceal bleeding, but physicians' knowledge of treatment guidelines is limited.
  • Two surveys were conducted among Austrian gastroenterologists to assess their understanding of variceal management and the use of albumin, finding mixed adherence to recommendations.
  • Results revealed a tendency for overtreatment, with 30% recommending unnecessary follow-up procedures, while most specialists were unclear on correct indications for early interventions and demonstrated inconsistencies in albumin dosage adherence for various conditions.

Endoscopic ultrasound (EUS) is a very sensitive examination to detect pancreatic masses and can provide useful information in cases where conventional radiologic workup remains inconclusive. We present three cases in which EUS was decisive in establishing the correct diagnosis. Case 1: A 74-year-old female was hospitalized because of acute pancreatitis.

Ultrasound elastography including transient elastography (TE), point shear wave elastography, (pSWE) and two (three)- dimensional shear wave elastography (2D-SWE) have been introduced mainly for the evaluation of the liver. All the techniques are also feasible for the examination of spleen, whereas pSWE and 2D-SWE can be used for the assessment of the pancreas, kidney, gastrointestinal tract and other organs. Strain elastography also plays a role for non-liver applications.

Background: Platelets have been reported to influence tumor biology and may promote metastasis. Traditionally, thrombocytopenia, a hallmark of cirrhosis, was associated with hepatocellular carcinoma (HCC) development. However, the impact of platelet count on outcome in patients with established HCC is not well studied.

Background/aims: von Willebrand factor antigen (vWF-Ag) is a noninvasive predictor of portal hypertension that serves as a negative prognostic marker in various malignancies. Increased portal hypertension is associated with higher postoperative morbidity and decreased survival after hepatectomy. The purpose of this study was to determine the correlation between vWF-Ag, postoperative morbidity and oncological outcome.

Article Synopsis
  • The study aimed to analyze the connection between shear wave velocity in the kidneys of patients with chronic glomerulonephritis (CGN) and histological parameters from renal biopsies using Virtual Touch Quantification (VTQ).
  • The research included 20 patients with CGN and 57 normal controls, measuring kidney stiffness and assessing histological features like glomerulosclerosis and interstitial fibrosis.
  • Results indicated that shear wave velocities were significantly lower in CGN patients compared to controls, and lower VTQ values were associated with interstitial fibrosis and arteriolo-hyalinosis, suggesting a link between kidney stiffness and disease severity.

In the last 10 years the availability of ultrasound elastography allowed to diagnose and stage liver fibrosis in a non-invasive way and changed the clinical practice of hepatology. Newer ultrasound-based techniques to evaluate properties of the liver tissue other than fibrosis are emerging and will lead to a more complete characterization of the full spectrum of diffuse and focal liver disease. Since these methods are currently undergoing validation and go beyond elastography for liver tissue evaluation, they were not included in the recent guidelines regarding elastography issued by the European Federation of Societies in Ultrasound in Medicine and Biology.

In previous studies of acoustic radiation force impulse (ARFI) elastography, using Virtual Touch tissue quantification (VTQ) (Siemens Acuson S2000), it was reported that the measurement of renal shear wave speed in patients with chronic kidney disease (CKD) is not influenced exclusively by renal fibrosis. The purpose of the present study was to analyze the role of VTQ in patients with diabetic kidney disease, considered the main cause of CKD. The study group included 164 patients: 80 patients with diabetic kidney disease (DKD) and 84 without renal disease or diabetes mellitus.
