A 66-year-old man, who was a passenger in a car involved in a low-speed head-on motor vehicle accident, was rushed to our hospital. His abdomen was tender and distended. An enhanced computed tomography scan showed a massive retroperitoneal hematoma, and its three-dimensional imaging revealed an active leak of the contrast medium from the aortic bifurcation.

In addition, based on a platelet count of 172 x 10(4)/mm(3) as well as the characteristic bone marrow findings, he was diagnosed to have essential thrombocythemia. After the platelet count and aggregation response improved on administration of ranimustine and antiplatelet agents, infrainguinal bypass surgery was performed using the saphenous vein and prosthetic grafts.

A medial approach was used to operate on the left leg, and the vein bypass was made from the superficial femoral artery to the crural artery through a subfascial route. A posterior approach was used to operate on the right leg and it was found that the mid-popliteal artery passed medial to and beneath the medial head of the gastrocnemius muscle and was severely compressed by an accessory slip of muscle.
