: There remain several challenges to the acceptance and standardization of active surveillance (AS) in patients with early thyroid cancer. The purpose of this study was to update the evidence of tumor progression outcomes in AS to support resolution of clinical concerns and to examine the effect of follow-up duration, period context such as diagnostic techniques, and tumor size at baseline on outcomes. : PubMed, Web of Science, and Embase were used to search for original articles in English until January 2024.

Article Synopsis
  • - The study aimed to compare the perioperative and oncological outcomes of hand-assisted laparoscopic nephroureterectomy (HANU) with robotic-assisted nephroureterectomy (RANU) among matched patients.
  • - Both groups included mostly elderly patients (average age around 70), but RANU showed benefits like less blood loss, quicker recovery of oral intake, and shorter hospital stays compared to HANU, although RANU patients reported more wound pain.
  • - Overall, both surgical methods produced similar operative and early postoperative results, but more extended follow-up is needed to evaluate their long-term oncological outcomes.

N-methyl-D-aspartate (NMDA) receptor activation in rat kidney reduces renal perfusion and ultrafiltration. Hypoperfusion-induced ischemia is the most frequent cause of functional insufficiency in the endotoxemic kidney. Here, we used non-hypotensive rat model of lipopolysaccharide-induced endotoxemia to examine whether NMDA receptor hyperfunction contributes to acute kidney injury.

Background: To report Taiwan's experience in robot-assisted laparoscopic nephroureterectomy (RANU) for upper tract urothelial carcinoma (UTUC).

Methods: Twenty patients with a diagnosis of renal pelvic or ureteral urothelial carcinoma underwent RANU at three medical centers. We performed RANU by re-docking the robot after the nephrectomy with or without repositioning for excision of the distal ureter and bladder cuff.

Background/objectives: Urinary stones (US) are associated with systemic metabolic and endocrine disorders that share risk factors typically associated with acute coronary syndrome (ACS).

Methods: For this investigation, 30,142 patients with US were set as the research group, and 121,768 randomly selected patients were set as the comparison group through frequency matching by age, sex, and index year. Each patient was individually tracked to identify those who developed ACS during the follow-up period.

Article Synopsis
  • Chronic inflammation, indicated by serum C-reactive protein (CRP) levels, may play a role in the lower urinary tract symptoms (LUTS) experienced by men with benign prostatic hyperplasia (BPH).
  • The study involved 853 men, examining factors such as age, prostate volume, and CRP levels in relation to urinary symptoms, revealing significant associations between CRP levels and symptoms like urgency and nocturia.
  • After treatment with medications like tolterodine, patients showed a notable decrease in CRP levels, especially those with a specific balance of urinary symptoms, suggesting a link between inflammation and urinary function improvement.

We present the first report of LESS bilateral nephroureterectomy via the transperitoneal approach using the home-made single port. Two patients received LESS bilateral nephroureterectomies and bladder cuffs resection with homemade single ports, which were created by using an Alexis wound retractor as an access platform through a 4 cm incision. Distal ureters were resected through the same incision with Endo-loop.

Background/purpose: To evaluate the long-term oncological outcomes of hand-assisted retroperitoneoscopic radical nephrectomy (HARRN) for treating clinically localized renal cell carcinoma.

Methods: We retrospectively collected and analyzed the data and clinical outcomes of 46 patients who underwent HARRN and 50 patients who underwent conventional open radical nephrectomy (ORN) at our institution for clinical localized renal cell carcinoma (RCC).

Results: The median follow-up period of the HARRN group was 56.

Article Synopsis
  • Six LESS radical nephrectomies were carried out using a retroperitoneal approach with the Alexis wound retractor.
  • The surgery employed standard laparoscopic instruments for effective operation.
  • Results indicated that the RLESS technique is a safe and practical option for treating localized renal cancer.

The Hem-o-Lok clips are widely used in various laparoscopic operations because of its easy application and secure clamping. The Hem-o-Lok clips were adopted to ligate bladder cuff during bladder cuff excision because of its advantages. We report 2 cases of bladder migration of Hem-o-Lok clips after laparoscopic nephroureterectomy and bladder cuff excision.

Article Synopsis
  • This report evaluates the authors' experience with a minimally invasive surgical technique called laparoendoscopic single-site (LESS) totally extraperitoneal (TEP) inguinal hernia repair in 100 patients.
  • The study involved analyzing patient demographics, hernia specifics, and the outcomes of the procedure, noting a high success rate with only two conversions to different repair methods and one recurrence.
  • Results showed an average surgery time of 64.2 minutes with a short hospital stay of 1.54 days, indicating that the procedure is feasible and safe when performed by experienced surgeons, although some minor complications were reported.

Objective: To present the initial experience of laparoendoscopic single-site (LESS) nephroureterectomy via the transperitoneal approach using the Alexis wound retractor and bladder cuff resection by endoloop.

Methods: Two patients received LESS nephroureterectomies and bladder cuffs resection with homemade single ports, which were created by using an Alexis wound retractor as an access platform through a 4-cm incision. Distal ureters were resected through the same incision with endoloop.

Article Synopsis
  • The study evaluates how renal function impacts the success rate of extracorporeal shock wave lithotripsy (ESWL) in treating proximal ureteric stones (PUS), noting that urinary obstruction can worsen renal function.
  • It found that patients with an estimated glomerular filtration rate (eGFR) of 60 mL/min or higher had a significantly better stone-free rate (SFR) of 93%, compared to just 50% for those with lower eGFR.
  • Key predictors affecting SFR included eGFR ≥ 60, stone width, and gender, with a logistic regression model developed to predict the likelihood of being stone-free after treatment.

Article Synopsis
  • The study aimed to assess the effectiveness of laparoscopic adrenalectomy, a minimally invasive surgical technique, for treating potentially malignant adrenal cysts in eight patients over the past decade.
  • The procedure involved completely removing the adrenal gland and cysts using a laparoscopic approach, with no reported complications, a mean operation time of 135 minutes, and an average hospital stay of 3.4 days.
  • Follow-up examinations showed all patients remained symptom-free with no signs of recurrence after an average of 40 months, highlighting the safety and feasibility of this surgical method for complex adrenal cysts.

A 74-year-old man with hypertension presented with foamy urine and intermittent right upper quadrant abdominal pain. He had undergone 2 sessions of percutaneous nephrolithotripsy for right renal stones and shock wave lithotripsy 6 months before for residual stones. Abdominal computed tomography showed stones in an irregularly shaped right kidney and abscess formation that extended through the colon and was communicating with the duodenum.
