Article Synopsis
  • - Older adults (≥80 years) with acute myocardial infarction (AMI) experience more severe clinical symptoms and worse outcomes compared to younger patients (≤79 years), including higher rates of in-hospital mortality and major adverse cardiovascular events (MACE).
  • - Despite similar rates of primary percutaneous coronary intervention (PCI) between the age groups, frail patients aged over 80 showed significantly poorer clinical outcomes than their non-frail counterparts.
  • - Early invasive interventions like PCI may improve short-term survival in octogenarians and nonagenarians, even though they present with more severe conditions compared to younger patients.

Article Synopsis
  • A 41-year-old man diagnosed with idiopathic interstitial pneumonia and pulmonary hypertension presented symptoms that resembled pulmonary veno-occlusive disease (PVOD) despite a non-autoimmune background.
  • Previous lung biopsies did not show signs of venous occlusion, leading to a treatment with a phosphodiesterase type-5 inhibitor, which unexpectedly resulted in sudden pulmonary edema.
  • An autopsy revealed interstitial fibrosis and lobular septal vein occlusion, highlighting that pulmonary hypertension caused by interstitial fibrosis can mimic PVOD, necessitating careful diagnosis and treatment.

Article Synopsis
  • An 84-year-old woman with worsening shortness of breath was diagnosed with heart failure, which was indicated by her electrocardiogram and elevated B-type natriuretic peptide levels.
  • During treatment, she suffered from acute aortic thromboembolism and left atrial thrombus, leading to emergency surgeries to remove the clots.
  • A biopsy revealed amyloid deposits, confirming she had transthyretin cardiac amyloidosis, suggesting that her condition increased her risk of thrombosis even while in sinus rhythm.

Article Synopsis
  • - A 63-year-old man with hypertrophic cardiomyopathy experienced cardiac arrest from sustained ventricular tachycardia and received an implantable cardioverter-defibrillator (ICD) for treatment.
  • - After multiple episodes of ventricular tachycardia and a refractory electrical storm, he underwent successful epicardial catheter ablation but faced recurrence, leading to surgical intervention.
  • - For patients with hypertrophic cardiomyopathy and apical aneurysms, while ICDs are essential for preventing sudden death, surgical resection of apical aneurysms proves to be more effective than alternative treatments for managing refractory electrical storm.

Article Synopsis
  • Cardiogenic unilateral pulmonary edema (UPE) is linked to severe mitral regurgitation (MR) but its prevalence was not well-known prior to this study.
  • Among 38 patients with acute severe MR due to chordal rupture, 50% experienced UPE, primarily on the right side, while 21% had bilateral pulmonary edema (BPE).
  • Chest X-rays taken within 48 hours of symptoms identified UPE in 79% of cases, indicating that UPE is common shortly after chordal rupture and severe MR develops.

Acute coronary syndrome (ACS) with occlusion of the left circumflex coronary artery (LCX) poses diagnostic problems that may lead to a delay in reperfusion. From a group of 1,269 consecutive patients with ACS, 138 patients with ACS due to LCX occlusion were analyzed for clinical, electrocardiographic, and angiographic presentations, as well as door-to-balloon (DTB) time. Electrocardiographic changes were classified into 4 patterns: ST-segment elevation in inferior/lateral leads (ST-E); ST-segment depression in V1-V4 (ST-D); no significant ST changes (No-ST); and others.

Article Synopsis
  • The study reviews four patients with fulminant lymphocytic myocarditis experiencing cardiogenic shock who were treated with the Impella CP device for heart support.
  • Two patients recovered fully, one requires a bi-ventricular assist device while awaiting a transplant, and one patient died from multiple organ failure after treatment.
  • The authors suggest that predicting recovery and weaning from Impella support may rely on factors like the absence of right ventricular dysfunction and the severity of cardiac damage as indicated by biomarkers and other measurements.

Although atrial fibrillation (AF) is a well-known risk factor for embolic stroke in hypertrophic cardiomyopathy (HCM), there is a paucity of information derived from HCM patients who have experienced embolic stroke. From 141 consecutive HCM patients who had been hospitalized between 2000 and 2018, the clinical characteristics and management of 86 patients with AF were analyzed retrospectively. The incidence of embolic stroke was 36% (n=31 patients).

The impact of lesion calcium on long-term outcomes after drug-eluting stent implantation has not been adequately addressed. In 10,595 patients (16,803 lesions) who were exclusively treated with sirolimus-eluting stents in the j-Cypher registry, 5-year outcomes were compared between patients with ≥1 lesion with moderate or severe calcification (the calcium group) and those with noncalcified lesions only (the noncalcium group). Analyses were stratified by hemodialysis (HD) status (non-HD stratum [calcium n = 3,191, noncalcium n = 6,824] and HD stratum [calcium n = 415, noncalcium n = 165]).

It remains unclear whether mutational patterns of the hepatitis B virus (HBV) genome are associated with the development of severe hepatitis after the emergence of tyrosine-methionine-aspartate-aspartate (YMDD) variants during lamivudine treatment. Thirty patients with chronic hepatitis B who had YMDD variants during lamivudine therapy and were followed up subsequently while receiving lamivudine alone for at least 6 months were examined retrospectively. The lamivudine resistant mutations in the HBV polymerase gene were detected by a line probe assay, and the full-length sequences of HBV DNA were determined in some patients.
